GRP78、Ki67在胃癌中的表达及与临床参数之间的关系

摘　　要：目的 :应用免疫组织化学技术检测胃癌组织中葡萄糖调节蛋白78(GRP78)及Ki67表达水平,探讨二者与临床各参数之间的关系,二者的相关性及与胃癌疗效和预后的关系。方法:选择胃癌D2术后患者172例手术标本,先制成组织芯片,通过免疫组织化学方法检测GRP78、Ki67的表达,结合临床病理特征及随访资料进行综合分析。结果:172例胃癌组织中GRP78和Ki67表达阳性者分别为117例(68.02%)和103例(59.88%)。GRP78的表达与浸润深度、分化程度、淋巴结转移、临床分期、脉管癌栓有关,Ki67与浸润深度、分化程度、淋巴结转移、临床分期有关(P<0.05)。GRP78与Ki-67的表达呈正相关(r=0.5025,P<0.05)。GRP78及Ki67共表达阳性患者的总生存期低于共表达阴性者,差异有统计学意义(P<0.05)。GRP78表达阳性患者使用两药及以上联合化疗方案对比氟尿嘧啶类单药方案总生存时间的差异有统计学意义(P<0.05)。结论:GRP78、Ki67与胃癌各临床参数间关系密切,且二者表达呈正相关,对于胃癌D2术后辅助化疗的总生存有预测意义;GRP78对辅助化疗方案的选择具有判断价值。Objective：Application immunohistochemistry technique to detect gastric cancer tissue GRP78 and Ki67 expression level, and to explore the relationship between the parameters and clinical, understand the relevance of both,analysis on patients with positive expression of GRP78 in different auxiliary overall survival after treatment. Methods： This study collected D2 172 cases of postoperative gastric cancer, first made into tissue microarray, surgical specimens of gastric carcinoma was detected by immunohistochemical method in the expression of GRP78 and Ki67 situation,comprehensive analysis combined with clinical pathological characteristics and follow-up data, understand the relevance of their expression in gastric cancer and the relationship between the expression and clinical parameters of both. Result：（1）GRP78 in 172 cases of gastric cancer tissue and the expression of Ki67 was respectively 117 cases（68.02%） and 103 cases（59.88%）, the two were differentially expressed in different degree of differentiation of gastric cancer（P〈0.05）;（2）The expression of GRP78 and infiltration depth, the degree of differentiation, lymph node metastasis, clinical stage, vascular tumor emboli,Ki67 and infiltration depth, the degree of differentiation, lymph node metastasis, clinical stage（P〈0.05）;（3）Patients with gastric cancer by nonparametric correlation analysis, Pearson correlation coefficient was 0.5025, P〈0.05. Think that GRP78 and the expression of Ki67 were positively correlated;（4）GRP78 and Ki67 were positive for overall survival in patients with less than a total of expressing negatie, the difference was statistically significant（P〈0.05）;（5）GRP78 expression in patients with positive combination chemotherapy drug use two or more solutions compare fluorouracil class single-agent adjuvant chemotherapy after total survival time was statistically difference.
