原发性失眠患者工作记忆的相关研究

摘　　要：目的探讨原发性失眠患者工作记忆的特点。方法采用匹兹堡睡眠质量指数评估50例原发性失眠患者（病例组）及与其人口学资料相匹配的50名健康志愿者（对照组）的睡眠质量，视觉客体、语音及空间工作记忆评估采用延迟匹配工作记忆测试；采用独立样本t检验和Pearson相关分析比较2组的差异性及相关性。结果病例组的客体工作记忆错误数和空间工作记忆反应时[（5.24±2.44）个，（1 392.10±286.40） ms]与对照组[（4.18±2.31）个，（1 269.78±229.76） ms]比较差异有统计学意义（t=2.232，P=0.028；t=2.356, P=0.020）；匹兹堡睡眠质量指数得分与空间工作记忆反应时显著相关（r=0.487, P〈0.01）。结论原发性失眠患者存在客体工作记忆和空间工作记忆损害，空间工作记忆损伤程度与睡眠质量相关。Objective To explore working memory performance in patients with primary insomnia. Methods 50 patients with primary insomnia,well matched with 50 healthy controls in both gender and age were assessed by Pittsburgh Sleep Quality Index, and delayed match working memory task which included visual-object, visual-verbal and visual-spatial tasks. The difference between groups was analyzed by independent-samples t test. Meanwhile, Pearson correlation analysis was performed to further assess the correlation between working memory performance and the Pittsburgh Sleep Quality Index in patients with primary insomnia. Results The error number of object working memory task and reaction time of spatial working memory task in primary insomnia group （（5.24±2.44） time,（1 392.10± 286.40） ms） were significantly impaired than healthy controls group （（4.18±2.31） time,（1 269.78±229.76） ms, t=2.232, P=0.028; t=2.356, P=0.020）.Moreover, Pittsburgh Sleep Quality Index significantly correlated with the reaction time of spatial working memory task （r=0.487, P〈0.01）. Conclusion These data indicates that insomniac patients may suffer impaired working memory, especially the object working memory and spatial working memory. And the spatial working memory may strongly correlate to the sleep quality.
