摘 要:目的探讨双相障碍抑郁期、缓解期患者基底节豆状核脑生化代谢物水平变化特点。方法对29例双相障碍抑郁期患者(双相障碍抑郁组)、22例双相障碍缓解期患者(双相障碍缓解组)和24名年龄、性别、受教育年限与之匹配的健康对照者(对照组),应用磁共振氢质子波谱技术检测3组基底节区豆状核N-乙酰天冬氨酸(N-acetylasparte,NAA)、胆碱和肌酸3种代谢物水平,分别计算NAA/肌酸和胆碱/肌酸的比值。采用Pearson相关分析对双相障碍抑郁期、缓解期临床特征与豆状核磁共振氢质子波谱技术检测结果进行相关分析。结果双相障碍抑郁组(1.5±0.2、1.5±0.1)、双相障碍缓解组(1.5±0.1、1.5±0.1)左侧、右侧豆状核NAA/肌酸值低于对照组(1.7±0.3、1.7±0.2),3组间差异均有统计学意义(F=5.614,P=0.005;F=7.811,P=0.001)。3组间双侧豆状核胆碱/肌酸值,差异均无统计学意义。经Pearson相关分析显示,双相障碍抑郁期、缓解期患者临床特征(年龄、总病程、发作次数、汉密尔顿抑郁量表和杨氏躁狂量表评分)与脑生化代谢物水平比值不相关。结论双相障碍抑郁期及缓解期患者均存在双侧基底节豆状核神经元代谢功能降低,豆状核神经元功能下降可能是双相抑郁的神经影像学内表型。Objective To identify the possible alteration of brain functioning metabolism levels of the lenticular nuclei in basal ganglia in patients with bipolar depressive episode and euthymic state using proton magnetic resonance spectroscopy (1H-MRS).Methods The N-acetylaspartate(NAA), choline(Cho), and creatine(Cr) of the lenticular nucleus in basal ganglia were studied in 29 bipolar disorder (BD) patients during acute depressive episode, 22 euthymic BD patients and 24 age-, sex-, and educational-matched right-handed healthy controls. The ratios of NAA/Cr and Cho/Cr were calculated. Pearson's correlation analyses were used to determine whether clinical variables correlated to the brain biochemical metabolite ratios in both acute and euthymic patients with BD.Results On the comparison of biochemical changes, both acute (1.5±0.2, 1.5±0.1) and euthymic (1.5±0.1, 1.5±0.1) BD patients showed significantly lower NAA/Cr ratios in bilateral lenticular nucleus when compared to the healthy controls (1.7±0.3, 1.7±0.2; F=5.614, P=0.005; F=7.811, P=0.001). There were no significant differences among 3 experimental groups for Cho/Cr ratiain in bilaterdl lenticular nucleus. On the comparison of correlation of brain biochemical changes and six clinical variables, there was no significant correlation between two metaboliteratios and age, duration of illness number of episodes. 24-item HAMD and YMRS scores.Conclusions These findings suggest that biochemical abnormalities in the lenticular nucleus may occur both in acute and euthymic BD patients. Metabolic decline in bilateral lenticular nucleus likely are markers of endophenotypes of BD.
