牙周基础治疗对合并有糖尿病的患者治疗价值分析

摘　　要：目的探讨牙周基础治疗对合并有糖尿病的患者治疗价值。方法 40例慢性牙周炎合并2型糖尿病患者作为观察组,40例无糖尿病背景的慢性牙周炎患者作为对照组,两组均给予牙周基础治疗。比较两组牙周情况及各生化指标的变化。结果两组患者治疗前齿龈沟出血指数(SBI)、探诊深度(PD)、C反应蛋白(CRP)、肿瘤坏死因子-α(TNF-α)水平比较差异均无统计学意义(P>0.05)。治疗后两组患者SBI、PD、CRP、TNF-α水平均较治疗前显著降低,差异有统计学意义(P<0.05),且观察组高于对照组,差异有统计学意义(P<0.05)。观察组治疗后的空腹血糖(FPG)、糖化血红蛋白(Hb A1c)水平[(9.84±1.35)mmol/L、(8.43±1.33)%]低于治疗前[(12.54±2.38)mmol/L、(9.48±1.67)%],差异有统计学意义(P<0.05)。结论牙周基础治疗能够显著改善慢性牙周炎合并糖尿病患者的牙周情况,降低患者的血糖水平。Objective To explore therapeutic value of periodontal initial therapy for diabetic patients. Methods A total of 40 chronic periodontitis patients with type 2 diabetes were named as observation group, and 40 chronic periodontitis patients without diabetes as control group. Both groups received periodontal initial therapy. Comparison were made on provisional restoration and changes of biochemical indexes in two groups. Results Both groups had no statistically significant difference in gingival sulcus bleeding index（SBI）, probing depth（PD）, C-reaction protein（CRP） andtumor necrosis factor-α（TNF-α） before treatment（P〈0.05）. Both groups had lower level of SBI, PD, CRP and TNF-α after treatment than before treatment, and differences had statistical significance（P〈0.05）, but the observation group had higher indexes than the control group, and differences had statistical significance（P〈0.05）. The observation group had lower level of fasting plasma glucose（FPG） and glycosylated hemoglobin（Hb A1c） [（9.84±1.35） mmol/L and（8.43±1.33） %] than treatment before[（12.54±2.38） mmol/L and（9.48±1.67） %],and the difference had statistical significance（P〈0.05）. Conclusion Periodontal initial therapy can significantly improve provisional restoration of chronic periodontitis patients with diabetes, and decrease blood glucose level of patients.
