盐酸文拉法辛合并小剂量奥氮平对伴有躯体症状抑郁症的对照研究

摘　　要：目的分析使用盐酸文拉法辛以及小剂量奥氮平治疗伴有躯体症状抑郁症的临床效果。方法 80例伴有躯体症状抑郁症患者,依照盲选法分为试验组与对照组,每组40例。试验组使用盐酸文拉法辛合并小剂量奥氮平治疗,对照组仅使用盐酸文拉法辛治疗,均治疗6周。对比两组治疗前、治疗6周后的抑郁症状评分及治疗后的临床疗效。结果经6周治疗,试验组总有效率92.50%高于对照组67.50%,差异具有统计学意义(P<0.05)。试验组与对照组治疗前汉密尔顿抑郁量表(HAMD)评分分别为(31.1±2.5)、(31.3±2.4)分,治疗6周后分别为(15.2±1.5)、(20.3±1.9)分。两组治疗前HAMD评分比较差异无统计学意义(P>0.05);治疗后两组HAMD评分均前治疗较明显降低,且试验组低于对照组,比较差异均具有统计学意义(P<0.05)。结论对伴有躯体症状抑郁症患者使用小剂量奥氮平以及盐酸文拉法辛联合治疗,可有效提升治疗效果,促进患者恢复。Objective To analyze clinical effect of venlafaxine hydrochloride combined with small dose of olanzapine in the treatment of depression accompanied by physical symptoms. Methods A total of 80 patients with depression accompanied by physical symptoms were divided by blind selection method into experimental group and control group, with 40 cases in each group. The experimental group received venlafaxine hydrochloride combined with small dose of olanzapine for 6-week treatment, and the control group received venlafaxine for 6-week treatment. Comparison were made on depression symptom score and clinical effect of before treatment,after 6-week treatment. Results After 6-week treatment, the experimental group had higher total effective rate as 92.50% than 67.50% in the control group,and the difference had statistical significance（P〈0.05）. The experimental group and control group had Hamilton depression scale（HAMD） respectively as（31.1±2.5） and（31.3±2.4） points before treatment,（15.2±1.5） and（20.3±1.9） points after 6-week treatment. There was no statistically significant difference of HAMD score in two groups（P〈0.05）; Both groups had lower HAMD score than treatment before, and the experimental group had lower score than the control group, and differences all had statistical significance（P〈0.05）. Conclusion Venlafaxine hydrochloride combined with small dose of olanzapine in the treatment of depression accompanied by physical symptoms can effectively improve treatment effect and promote recovery of patients.
