摘 要:目的:对内蒙古牧区蒙古族细菌性阴道病(Bacterial Vaginosis,BV)病人和健康妇女阴道内乳杆菌及局部分泌型免疫球蛋白A(secretary immunoglobulin A,SIgA)含量进行研究,探讨蒙古族妇女阴道内乳杆菌与生殖道免疫状况的相关性,为阴道微生态平衡与阴道黏膜免疫屏障关系的研究提供理论依据,并进一步丰富民族地区阴道微生态理论。方法:选取2014-03~2015-03间45名蒙古族健康妇女、37名蒙古族BV病人,采集阴道分泌物行乳杆菌的分离、培养、鉴定、产H_2O_2能力检测,联合放射免疫双抗体夹心法测定阴道分泌液中SIgA含量。结果:蒙古族健康妇女阴道内乳杆菌检出率及产H_2O_2乳杆菌分离率明显高于BV病人(P〈0.05),两组主要菌种分布无统计学差异(P〉0.05),BV病人阴道内局部SIgA含量明显高于健康妇女(P〈0.05)。结论:H_2O_2阳性乳杆菌是健康妇女阴道内的重要成员,BV病人阴道内乳杆菌数量、产H_2O_2能力及局部免疫功能发生显著改变,乳杆菌对生殖道黏膜免疫状况具有调节作用,而生殖道免疫因子变化也会对阴道菌群有影响作用。Objective: Study on Lactobacillus and SIgA Content between the women of mongol nationality with bacterial vaginosis and healthy women of mongol nationality in Inner Mongolia pasture,To investigate the relationship of Vaginal Lactobacillus and reproductive immunity in the Mongolian women; provide theoretical foundation for studying the relationship between vaginal microecological balance and vaginal mucosal immune barrier; and further enrich the theory of vaginal microecological in ethnic minority areas. Methods: 45 Mongolian healthy women and 37 Patients of mongol nationality with bacterial vaginosis were selected during March 2014 to March 2015. The vaginal secretions were collected to isolation,culture,identification and detect the ability to product H_2O_2 of Lactobacillus;Combined radioimmunoassay double antibody sandwich method to detect the SIgA content in vaginal secretions. Results: The detection rates of Lactobacillus and ability to product H_2O_2 in the vaginas of Mongulia healthy women were significantly higher than patients with bacterial vaginosis( P〈0. 05),There was no significant difference in distribution of major lactobacillus species between the two groups( P〈0. 05). The SIgA Content in the vagina of patients with bacterial vaginosis was significantly higher than healthy women( P〈0. 05). Conclusion: H_2O_2-positive Lactobacillus is the important member in healthy women vagina. The number,ability to product H_2O_2 and Local immune function of Lactobacillus in patients with bacterial vaginosis have Changed significantly. Lactobacillus can regulate the immune status of the reproductive tract mucosal,while changes in the reproductive tract immune factors also affect the vaginal flora.
