HPV检测、传统涂片及液基制片法检测筛查宫颈癌结果比较研究

摘  要:目的比较人乳头瘤病毒(HPV)检测、传统巴氏涂片(CPS)及薄层液基细胞学(TCT)检测在宫颈癌筛查中的应用价值。方法选取682例宫颈癌筛查者作为研究对象,对所有对象进行HPV、CPS及TCT检测,并以宫颈组织活检结果作为金标准,比较上述3种方法检测的准确性。结果 682例宫颈癌筛查者中,经组织病理学检查阳性者101例(14.81%)。HPV的阳性似然比、阴性似然比均明显高于TCT、CPS;HPV、TCT的敏感度、准确度、阴性预测值均大于CPS,HPV、CPS的特异度、阳性预测值均大于TCT,3种筛查方法的敏感度、特异度、准确度、阴性预测值比较有统计学意义(P<0.01)。结论 TCT与HPV在宫颈癌的筛查中准确性均较高,但HPV检测相对于TCT更具有可行性,适宜于在基层医院推广。Objective To compare the application value of Human papillomavirus( HPV) detection,conventional Pap smear( CPS) and thin-layer liquid-based cytology( TCT) detection in cervical cancer screening. Methods 682 cases of cervical cancer screening were selected as the study object,all subjects were tested for HPV,CPS and TCT detection,the accuracy of the above three methods of detection were compared by the results of cervical biopsy as the gold standard. Results682 cases of cervical cancer screening,101 cases( 14. 81%) were positive by histopathological examination. HPV positive likelihood ratio and negative likelihood ratio were significantly higher than TCT,CPS; HPV,TCT sensitivity,accuracy,negative predictive values were greater than CPS,HPV,CPS specificity,positive predictive values were greater than TCT,the sensitivity,specificity,accuracy and negative predictive value of three screening methods were statistically significant( P〈0. 01). Conclusion The accuracy of TCT and HPV in the screening of cervical cancer is higher,but the HPV detection is more feasible than the TCT,is suitable for promotion in primary hospitals.
