陕西省渭南地区妇女人群人乳头瘤病毒感染状况及亚型分布特征

摘　　要：目的探讨陕西省渭南地区妇女人群人乳头瘤病毒（papillomavirus,HPV）感染及其基因亚型的分布特征。方法采用PCR反向点杂交技术对陕西省渭南地区常住妇女4 329人进行HPV感染分型检测。结果陕西省渭南地区妇女人群HPV总体感染率为25.43%（1 101/4 329）,高危型感染率为23.75%（1 028/4 329）,低危型感染率为0.30%（13/4 329）,高低危混合感染率为1.39%（60/4 329）。最常见总体、单一和多重感染亚型均依次为16、52、58、53、18和33型。不同年龄段妇女人群HPV常见亚型分别依次为6、58、16、52、33和51型,52、16、51、58、68和33型,16、52、53、18、33和58型,16、58、52、18、53和31型,16、58、52、33、56和53型,58、53、16、33、52和18型。其各年龄段的感染率以小于20岁年龄段为最高,达51.94%（67/129）,其次为大于60岁年龄段（34.38%,77/224）,以31~40岁年龄段为最低（20.79%）。总体、高危和低危亚型感染率在不同年龄段均有较大差异（P〈0.05）,而高低危混合感染率在各年龄段则差异无统计学意义（P〉0.05）。单一和多重感染率均以小于20岁年龄段为最高（46.51%和10.85%）,其次是大于60岁年龄段（24.11%和7.14%）,以31~40岁年龄段为最低（15.26%和5.38%）。在多重感染中以二重感染最为常见,占多重感染85.55%,其二重感染常见类型为16、58和52、53型。结论陕西省渭南地区妇女人群HPV感染率较高,高低危型分别以16、52、58、53、18、33和6、81型比较常见。不同年龄段的亚型均有不同分布特征。总体、高危和低危感染率在不同年龄段均有较大差异。Objective To study human papillomavirus（HPV）infection and gene types among women in Weinan district of Shaanxi province in China. Methods Cervical cell specimens from a total of 4 329 cases were collected and 23 HPV genotypes were analyzed by PCR and reverse dot blot hybridization. Results The total,high-risk,low-risk and h-l-HPV infection positive rates were 25.43%,23.75%,0.30% and 1.39%,respectively.The most common subtypes of total,single and multiple HPV infections were subtypes 16,52,58,53,18 and 33.Different age groups and the corresponding common subtypes were subtypes 18-20 year group（6,58,16,52,33,51）,21-30 year group（52,16,51,58,68,33）,31-40 year group（16,52,53,18,33,58）,41-50 year group（16,58,52,18,53,31）,51-59 year group（16,58,52,33,56,53）and ages〉60group（58,53,16,33,52,18）,respectively.There are significant differences in the infection rates among different age groups（P〈0.05）with the highest at〈20years old group（51.94%）and next〉60years old group（34.38%）;the lowest was at 31-40 years old group（20.79%）.There is no obvious difference in the infection rates of high and low risk HPV mixed infections among all age groups（P〉0.05）.Single and multiple infection rates were highest in 〈20age group（46.51% and 10.85%）and the second at 〉60age group（24.11%and 7.14%）while the lowest was at 31-40 years old group（15.26%and 5.38%）.The double infection was most common（85.55%）in all multiple infections and its common genotypes were 16,58 and 52,53 types respectively. Conclusions HPV infection rate of women in Weinan district of Shaanxi province was relatively high and common high-risk and low-risk genotypes were 16,52,58,53,18,33 and 6,81 types respectively.The subtype distribution of different age groups and the rate of total infection,high-risk and lowrisk HPV infection were significantly different in different age groups.
