多排螺旋CT在直肠癌术前诊断中的应用价值分析

摘　　要：目的探讨多排螺旋CT检查在直肠癌患者术前诊断的价值。方法 64例直肠癌患者,观察其术前进行多排螺旋CT检查结果,并与手术以及病理结果进行比较,评估多排螺旋CT检查效果。结果①本组18例患者多排螺旋CT诊断为Ⅰ期直肠癌,表现为肠腔内实质性肿块,无肠壁增厚,增强后肿块强化明显。②本组23例患者多排螺旋CT诊断为Ⅱ期直肠癌,表现为肠壁呈局限性增厚或者环形增厚,无周围软组织侵犯。③本组15例患者多排螺旋CT诊断为Ⅲ期直肠癌,表现为癌肿向直肠管壁周围侵犯。④本组8例患者多排螺旋CT诊断为Ⅳ期直肠癌,表现为邻近组织和器官受侵,直肠周围肌肉组织如提肛肌、梨状肌等受侵表现为粗细不均、模糊。通过与病理结果对照发现,多排螺旋CT检査的直肠癌分期诊断结果与病理诊断结果较为相符。结论多排螺旋CT检査对于直肠癌病变的诊断有着极其重要的价值,是直肠癌患者术前非常必要的检査手段,可依据扫描所得图像信息联合其他检査,进一歩明确诊断,并为判断预后提供准确依据,以及为治疗方案提供详尽参考。Objective To investigate value by multislice spiral CT in preoperative diagnosis of rectal cancer. Methods Examination outcomes by preoperative multislice spiral CT in 64 rectal cancer patients were observed and compared with surgical and pathological outcomes, Examination effect by multisliee spiral CT was evaluated. Results ① There were 18 cases with diagnosed stage I rectal cancer by multisliee spiral CT, with manifestation as enterocoel solid mass, no bowel wall thickening, and obvious mass reinforcement after enhancement. ② There were 23 cases with diagnosed stage II rectal cancer by muhislice spiral CT, with manifestation as local or annular bowel wall thickening and no soft tissue invasion. ③ There were 15 cases with diagnosed stage III rectal cancer by muhislice spiral CT, with manifestation as peripheral invasion from tumor to rectal wall. ④ There were 8 cases with diagnosed stage IV rectal cancer by multislice spiral CT, with manifestation as adjacent tissue and organ invasion, uneven and unclear involved levator ani and piriformis around rectum. Comparison with pathological outcome showed corresponding outcome by muhislice spiral CT in diagnosis of rectal cancer staging, Conclusion As an essential measure in examination before rectal cancer operation, multislice spiral CT contains important value in diagnosis of rectal cancer. Combination of scanning images and other examination can show further accurate diagnosis, and provide precise reference in prognosis judgment and detailed reference in treatment scheme.
