摘 要:目的分析对高摄取的炎性肿块和肿瘤病灶内^(18)氟-脱氧核糖(^(18)F-FDG)分布特征及可能机理。方法肺内类肿块性炎性病变患者28例(炎症组),明确肺癌患者25例(肿瘤组),所选患者^(18)F-FDG摄取的最大葡萄糖摄取值(SUVmax)均>2.5。获取每一病灶相关数据后导入MATLAB软件中,通过三维图像(surf图)观察炎症组和肿瘤组量化后的^(18)F-FDG值分布特点。结果28例炎性患者平均SUVmax值为6.74±2.25,对照组为8.20±2.54,两者间比较差异有统计学意义(P<0.05)。28例炎性患者中,18例(18/28,64.29%)病灶SUVmax所在平面FDG分布图像呈"双峰状"或"多峰状"。10例患者(10/28,35.71%)病灶图像呈"单峰状"。肿瘤组22例患者(22/25,88.00%)病灶^(18)F-FDG分布的图像呈"单峰状",3例患者(3/25,12.00%)病灶图像呈"双峰状"或"多峰状"。结论适当进行肺内肿块^(18)F-FDG分布规律的分析(双峰状或是单峰状),对于鉴定肺内肿块的良恶性具有优势。Objective To analyze the characteristics and mechanism of 18 F-FDG distribution on high metabolic inflammatory tumor-like lesions within the lung.Methods In this study,28 patients with inflammatory tumor-like lesions(inflammation group)and 25 patients with confirmed lung cancer(cancer group)were enrolled.The value of SUVmax of 18 F-FDG was larger than 2.5 in all patients.The distribu-tion of 18 F-FDG was evaluated by the surf map via MATLAB software.Results The average SUVmax in the inflammation group was significantly higher than that of the cancer group [(6.74 ±2.25)vs(8.20 ± 2.54),P 〈0.05].In the inflammation group,18 cases(64.29%)presented two peaks or multiple peaks and 10 cases(35.71%)presented one peak in the surf map for 18 F-FDG.In the cancer group,3 cases (12.00%)presented two peaks or multiple peaks and 22 cases(88.00%)presented one peak in the surf map for 18 F-FDG.Conclusion Distribution analysis of 18 F-FDG may have great advantages for the identi-fication of benign and malignant pulmonary masses.
