调神法对急性期面神经炎患者分型治疗的肌电图分析及远期疗效观察

摘　　要：目的利用肌电图检查评估调神法对不同轻重分型的急性期面神经炎患者的疗效差异及远期疗效观察。方法对符合诊断标准的57例急性期面神经炎患者给予调神法针刺治疗;发病5~7天进行肌电图检测,根据肌电图检查结果分型;根据Sunnybrook(多伦多)及House-Brackmann(H-B)面神经功能评价分级系统评定疗效,随访1年观察患者并发症出现情况。结果轻型、中型及重型患者治疗后Sunnybrook(多伦多)评分明显高于治疗前患者,轻型患者House-Brackmann(H-B)面神经功能评价明显优于中型和重型患者;中型患者评分明显优于重型患者,轻型患者痊愈及显效主要在第一及第二疗程,中型及重型主要分布在第二及第三疗程,轻型患者随访1年无并发症出现,重型并发症出现率明显高于中型患者。结论根据患者临床表现结合肌电图结果可对患者分型判断预后,轻型患者预后良好,治愈率高,一般不出现并发症,病情越重,肌电图提示神经损伤越重,则疗程越长,并发症出现率越高。Objective To evaluate the effect of electromyogram （EMG） on patients with acute facial neuritis in different severity categories by EMG and to observe the long-term therapeutic effect. Methods Electromyography was per- formed on 57 patients with acute facial neuritis in accordance with the criteria of diagnosis. According to the results of electromyography, the patients were divided. Sunnybrook （Toronto） and House-Brackmann （HB） facial nerve function evaluation grading system was applied to assess the treatment efficacy ; follow-up of 1 year was administrated to observe of patients with complications. Results The Sunnybrook score was significantly higher in mild, moderate, and severe patients than in pre-treatment patients, and the facial nerve function of House-Brackmann （HB） was significantly better in mild patients than in moderate and severe patients; the medium-sized patients were significantly better than those in severe patients. Patients were cured and markedly in the first and second courses, medium and heavy axe mainly distributed in the second and third courses, light patients were followed up for I year with no complications, the incidence of severe complications was significantly higher than medium- sized patients. Conclusion According to the clinical manifestation and the electromyogram, the prognosis of the patients can be classified. The prognosis of the patients is good and the cure rate is high. There is no complication in general. The electromyogram shows that the heavier the nerve injury is, The higher incidence of complications will occur.
