摘  要:目的分析帕金森病(PD)患者唾液中的α-突触核蛋白含量与纹状体多巴胺摄取功能的相关性,探讨其是否可能作为PD的早期生物学标志物。方法选取门诊确诊的原发性PD患者70例,根据发病年龄分为早发组(EOPD组,发病年龄≤50岁)25例和晚发组(LOPD组,发病年龄〉50岁)45例。应用Luminex技术检测并比较两组PD患者唾液α-突触核蛋白的水平,运用放射性核素^99m锝一巯胺托品标记的单光子发射计算机断层扫描(^99mTc-TRODAT-1 SPECT)检测PD患者脑内纹状体多巴胺摄取功能,并分析PD患者唾液α-突触核蛋白含量与纹状体多巴胺摄取功能的相关性。结果LOPD组患者唾液中α-突触核蛋白相对平均含量较EOPD组明显降低(t=2.833,P=0.005)。总体70例PD患者唾液中α-突触核蛋白含量与纹状体、壳核、尾状核多巴胺摄取功能均无相关性(P〉0.05);性别、年龄校正前,EOPD组患者唾液中α-突触核蛋白含量与壳核^99mTc—TRODAT-1摄取功能呈较弱的正相关(r=0.4082,P=0.0428),校正后无相关性(r=0.1329,P=0.5454);LOPD组唾液中α-突触核蛋白含量与纹状体壳核及尾状核^99mTc—TRODAT-1摄取功能均无相关性(P〉0.05)。结论PD患者唾液中α-突触核蛋白含量随年龄增加而下降,其与纹状体多巴胺摄取功能无明显相关性。Objective To explore whether salivary α-synuclein could be a potential biomarker for monitoring disease progression in Parkinson' s disease (PD) by evaluating the association between salivary α-synuclein concentration and nigrostriatal dopaminergic function. Methods Seventy consecutive outpatient clinic PD cases were recruited, of which were divided into early-onset PD group ( EOPD group, n = 14, age at onset ≤50 years old) and late-onset group (LOPD group, n = 56, age at onset 〉 50 years old). Firstly, saliva of PD patients were collected and α-synuclein levels in saliva were analyzed by quantitative and sensitive Luminex assay. Then, single photon emission computed tomography(SPECT) imaging with labeled dopamine transporters(DAT) (^99mTc-TRODAT-1) ,which has been used for measuring DAT density in PD was performed. Finally, we studied the relationship between saliva α-synuclein levels and nigrostriatal dopaminergic function. Results The salivary α-synuclein was significantly lower in LOPD group compared to EOPD group( t = 2. 833 ,P = 0. 005 ). No correlation was found between salivary a-synuclein levels and nigrostriatal uptake of ^99mTc-TRODAT-1 in total PD and LOPD patients, no matter controlled for age and gender or not (P 〉 0. 05 ). There was a slight correlation between salivary α-synuclein levels and putamen nucleus uptake of ^99mTc-TRODAT-1 in EOPD patients before adjustment for age and gender(r = 0. 4082, P = 0. 0428), but was no correlation after adjustment for age and gender ( r = 0. 1329, P = 0, 5454 ). Conclusion Salivary α-synuelein level decreases with age but has no correlation with nigrostriatal dopaminergic funetion.
