肺癌术后呼吸衰竭的临床分析

摘　　要：目的探讨肺癌术后发生呼吸衰竭的常见因素。方法选取郑州市第三人民医院2010-09—2016-06收治的28例肺癌术后发生呼吸衰竭的患者为研究对象,并同时选取同期手术治疗的62例肺癌术后未发生呼吸衰竭的患者作为对照组,采取临床统计分析。结果研究组患者的平均年龄、吸烟指数均显著高于对照组,差异有统计学意义(P<0.05);研究组术前各项肺功能指标均明显低于对照组(P<0.05);全肺切除术后呼吸衰竭发生率明显高于对照组(P<0.01)。结论呼吸衰竭是肺癌术后严重并发症之一,死亡率高,术后ICU加强监护治疗、及早给予机械通气是抢救成功的有效手段,同时探讨肺癌术后呼吸衰竭的原因及其特点,采取有效的防治措施在临床上具有重要意义。Objective To analyze the common factors causing postoperative respiratory failure in lung cancer surgery.Methods 28 cases with respiratory failure after lung cancer surgery admitted to Zhengzhou No. 3 People＇s Hospital from September 2010 to June 2016 were selected as the subjects of the research group.And 62 cases under- going lung cancer surgery without postoperative respiratory failure during the same period were selected as the control group.And clinical statistical analysis was made. Results The average age and smoking index in the research group were remarkably higher than those in the control group, with differences of statistical significance （P 〈 0.05）. The pre- operative lung function indexes in the research group were significantly lower than those in the control group（P〈 0.05）.The incidence rate of postoperative respiratory failure in sufferers with pneumonectomy was significantly higher than those in the control group（P〈0.01）.Conclusion Respiratory failure is one of the most critical complications with high mortality after lung cancer surgery, intensive care and treatment in ICU and early mechanical ventilation are effective measurements for successful rescue.So it is clinically important to explore the causes and features of respira- tory failure after lung cancer surgery and adopt effective control measures.
