黄芪多糖对肺癌外周循环MDSC的疗效影响

摘　　要：目的 探讨黄芪多糖对肺癌外周循环髓系抑制性细胞（peripheral circulating myeloid suppressor cells in lung cancer,MDSC）的影响及疗效。方法 选取晚期非小细胞肺癌患者100例,按随机数字表法分组,对照组50例予以单纯化疗,研究组50例在对照组基础上予以注射用黄芪多糖治疗,观察并记录2组间MDSC水平、亚群水平、血液精氨酸酶1（arginase-1,Arg-1）活性、肿瘤坏死因子α（tumor necrosis factorα,TNF-α）、白细胞介素-6（Interleukin-6,IL-6）水平,同时比较临床疗效及不良反应发生率。结果 与对照组比较,研究组治疗后MDSC（Lin-HLA DR-／CD33^＋／CD11b^＋）水平较低,与对照组比较治疗后研究组CD3^＋、CD4^＋及CD4^＋/CD8^＋水平较高,CD8^＋水平较低,与对照组比较治疗后研究组血清ARGⅠ活性、TNF-α、IL-6水平较低（P〈0.05）。对照组总有效率（42.0%）低于研究组总有效率（64.0%）,（P〈0.05）,对照组胃肠道反应（40/24）、3-4度白细胞减少（26/12）、肝功能损害（19/9）、肾功能损害（17/7）的发生率高于研究组（P〈0.05）。结论 黄芪多糖能够明显降低肺癌患者外周循环MDSC（Lin-HLA DR-／CD33^＋／CD11b^＋）水平,改善免疫功能,抑制血清ARGⅠ活性,降低TNF-α、IL-6水平,有效提高临床疗效,降低化疗毒副反应。Objective To investigate the effect of Astragalus Polysaccharide on peripheral circulation myeloid derived suppressor cells （MDSC） in lung cancer and its clinical effect. Methods 100 patients with advanced non small cell lung cancer were selected and divided into 2 groups, 50 cases in the control group treated with chemotherapy aalone, 50 cases in the experiment group received injection of Astragalus Polysaccharide on the basis of the control group, peripheral blood MDSC levels, peripheral blood T lymphocyte subsets levels, serum ARG I activity, TNF-a, IL-6 levels, the clinical effect and incidence of adverse reactions were compared after the treatment. Results Compared with the control group, peripheral blood levels of MDSC （Lin-HLA DR-／CD33、CD1 lb 4） was lower after treatment in the experiment group , peripheral blood levels o f CD3 ＇ CD4＋T lymphocytes and CD4＋/CD8＋ were higher, and CD8＋T lymphocytes level was lower after treatment, serum levels of ARG I activity, TNF-a, IL-6 levels were lower after treatment, （P〈0.05） , the total effective rate in the control group（42.0%）was lower than the experiment group（64.0%）, （P〈0.05）,the incidence of gastrointestinal reaction （40/24）,3 ？4 degree of leukocyte reduction（26/12）,liver fimction damage（19/9）, renal damage（17/7） in the control group were higher than the experiment group （P〈0.05）. Conclusion Astragalus Polysaccharide can significantly reduce the peripheral circulation MDSC （Lin-HLADRACDSSACDllb^ level in patients with lung cancer, improve T lymphocyte imimme function, inhibit the activity of serum ARG I, reduce the level of TNF-aand IL-6, effectively improve the clinical efficacy, and reduce the toxic and side effects of chemotherapy.
