新辅助化疗后肺癌患者淋巴结转移度及肿瘤N分期与患者生存质量的关系

摘　　要：目的探讨淋巴结转移度(LNR)及肿瘤N分期对新辅助化疗后肺癌患者生存质量及预后的影响。方法回顾性分析本院2006年1月-2013年12月行肺大部切除术并经病理确诊肺癌的患者110例,并进行随访。收集患者临床资料,采用线性回归模型探讨临床因素与患者生存质量的关系,采用Cox回归模型探讨LNR及肿瘤N分期与患者死亡的关系。结果 110例肺癌患者生存质量评分量表SF-36得分为58.9±10.7,在8个相关领域中,RE得分最高(63.9±12.4),BP得分最低(52.2±8.9);年龄、手术方式、LNR、N分期与患者生存质量明显相关(P<0.05);LNR、N分期、肿瘤侵犯深度是肺癌患者预后的独立危险因素(P<0.05),且LNR对肺癌患者的死亡预测价值更大,HR为12.9。结论 LNR、手术方式、肿瘤N分期、年龄皆为肺癌患者术后生存质量的重要影响因素,LNR对术后患者死亡的预测价值要优于肿瘤N分期。Objective To investigate the relationship of node metastasis ratio lymph（ LNR） and N stage with survival quality in lung cancer patients after neoadjuvant chemotherapy. Methods Retrospective analysis was performed on 110 patients who were diagnosed as lung cancer and underwent subtotal lung resection in our hospital from January 2006 to December 2013. Random surveillance was carried out. A linear regression model was used to investigate the relationship between the clinical data and the quality of life of patients according to the collected clinical data. Cox regression model was used to investigate the relationship of LNR and tumor N stage with death. Results SF- 36 score of 110 patients was 58. 9 ± 10. 7,the highest score was RE（ 63. 9 ± 12. 4）,and the lowest was BP（ 52. 2 ± 8. 9）; age,surgical procedure,LNR and N staging were significantly correlative with life quality of patients（ P〈0. 05）; LNR,N stage and tumor invasion depth were independent risk factors for the prognosis of patients with lung cancer（ P〈0. 05）. LNR had more prediction on mortality in patients with lung cancer,while HR was12. 9. Conclusion Age,surgical procedure,LNR and N stage are important factors that affect the life quality of patients with lung cancer,and the predictive value of LNR on postoperative mortality is better than that of N stage.
