度洛西汀联合心脑宁胶囊对晚发性抑郁症的疗效观察

摘　　要：目的:观察度洛西汀及心脑宁胶囊联合用药对晚发性抑郁症的认知功能作用和临床疗效。方法:根据入组排除标准选择2015年7月-2016年8月兰州大学第二医院门诊或住院晚发性抑郁症患者60例,限制性随机分组分研究组(度洛西汀联合心脑宁胶囊)和对照组(度洛西汀),治疗前与治疗第8周末采用蒙特利尔认知评估量表(Mo CA)和汉密尔顿抑郁量表(HAMD-24)分别评定认知功能和疗效,治疗前与治疗第1、2、4、8周末采用药物副反应量表评定不良反应。结果:治疗第8周末,两组Mo CA总分均高于治疗前,HAMD-24评分均低于治疗前,差异均有统计学意义(P<0.01);两组HAMD-24评分、痊愈率和有效率比较,差异均无统计学意义(P>0.05),研究组Mo CA总分高于对照组(P<0.01);两组均未见严重不良反应。结论:度洛西汀联合心脑宁胶囊与单用度洛西汀治疗晚发性抑郁症,安全性均较好,抗抑郁疗效相当,均能改善认知损害,但联合用药改善认知损害较度洛西汀单用明显。Objective： To investigate the efficacy and the cognitive amelioration on late-onset depression （ LOD ） of Duloxetine combined with Xinnaoning Capsule （ DCXC ） .Method： Via the restrictive random allocation method, out/in-patient clinic 60 patients inciuded in LOD from July 2015 to August 2016 in regard to the inclusion and exclusion criteria were divided into the research group with treatment of DCXC, and the control group with treatment of Duloxetine. The cognitive function and the clinical efficiency were determined respectively via Montreal Cognitive Assessment （ MoCA ） and Hamilton Depression Scale （ HAMD-24 ） before the treatment and after 8 weeks of treatment.And before the treatment and after 1, 2, 4, and 8 weeks of treatment, the safety was also done via Treatment Emergent Symptom Scale （ TESS ） .Result： MoCA scores were both higher and HAMD-24 scores were both lower after 8 weeks of treatment than those of before the treatment, the differences were both statistically meaningful （P〈0.01） .After 8 weeks of treatment, there were no significant differences in HAMD-24 scores, cure rate and effect rate （ P〉0.05 ） .And no serious side effect during the therapeutic between two groups （ P〉0.05 ） .However, MoCA score of the research group was remarkably higher than that of the control group （P〈0.01） .Conclusion： DCXC is pretty better than Duloxetine in the cognitive amelioration, as well similar in antidepression and safety.
