摘 要:目的:对脑出血患者的皮质下缺血性病变程度和血管性认知功能障碍的相关性进行系统性分析。方法:以江门市中心医院神经内科接收的69例脑出血患者作为研究对象,依照神经心理学测评差异分成对照组和受损组,对照组36例,受损组33例;对比两组患者的病灶直径、病灶数量、白质病变度及神经功能缺损评分(NIHSS评分和Bathel评分)情况,并通过回归分析法了解以上指标和患者认知功能障碍的相关性。结果:受损组患者额叶、基底核两区域的病灶直径、数量,白质病变度及NIHSS评分均要高于对照组,且Bathel评分低于对照组(P<0.05);通过回归分析表明,白质病变度和认知功能障碍存在正相关(P<0.05)。结论:通过判断脑出血患者皮质下缺血性病变程度可更准确地评估患者的认知功能障碍情况。Objective: To analyze the correlation between the degree of subcortical ischemie lesions and vascular cognitive impairment in pa- tients with cerebral hemorrhage. Methods : 69 cases of cerebral bemorrhage patients were selected as the research objects and divided into normal group and injury group in accordance with the neuropsychological evaluation differences, 36 cases and 33 cases respectively; Patients'lesion diame- ter, number of lesions, degree of white matter lesion and neurological fimetion defect score ( NIHSS score and Bathel score) were compared between the two groups, and regression analysis method was adopted to analyze the correlation between the above indexes and patients'cognitive impairment. Results:The patients'lesion diameter, number of' lesions, degree of white matter lesion and N IHSS score in frontal and basal nuclei in injury group were higher than those in normal group ( P 〈 0.05 ). There was positive correlation between the degree of white matter lesions and cognitive im- pairment ( P 〈 0.05 ). Conclusion: The degrees of subcortical ischemie lesions in cerebrnl hemorrhage patients can help to grasp patients'cogni- tive function impairment more accurately.
