胃癌组织中肿瘤浸润淋巴细胞与临床病理特征的相关性及预后的影响

摘　　要：目的观察胃癌组织中肿瘤浸润淋巴细胞（TILs）的表达，探讨其与胃癌临床病理特征的相关性及预后的影响。 方法利用光镜对956例胃癌组织中的TILs进行形态学观察。应用χ2检验分析TILs与临床病理特征的相关性，采用Kaplan-Meier生存分析和Cox回归模型分析TILs与患者生存期的关系。 结果在435例（45.5%）胃癌组织中观察到高表达的TILs，并具有特殊的组织病理形态特征。TILs的表达与肿瘤的TNM分期（P＝0.000）、分化程度（P＝0.000）、浸润深度（P＝0.000）、大小（P＝0.002）、淋巴结转移（P＝0.000）、神经侵犯（P＝0.005）和血管侵犯（P＝0.003）有关。高表达TILs的胃癌患者预后较好，中位生存期为（69.96±2.12）个月，低表达TILs患者预后较差，中位生存期为（54.78±1.99）个月（P＝0.000）。在多因素Cox回归模型中TILs[风险比（HR）＝0.815，P＝0.030]、患者年龄（HR＝1.496，P＝0.000）、TNM分期（HR＝1.756，P＝0.000）、血管内癌栓（HR＝1.389，P＝0.001）和淋巴结转移（HR＝1.864，P＝0.000）为独立预后因素。 结论胃癌组织中TILs可对机体起到重要的保护作用。Objective To investigate the correlation between tumor infiltrating lymphoeytes （TILs） and clinical pathological characteristics in gastric carcinoma and the effect on the prognosis. Metb- 2 otis Pathological changes of TILs in 956 cases of gastric cancer were observed by optical microscope. X test was used to analyze the correlation between TILs and the elinicopathological features, and Kaplan - Meier survival analysis and Cox regression model to analyze the relationship between the TILs and the prog- nosis of patients. Results In 435 cases （45. 5% ）, TILs was observed the high degree infiltrating in gas- trie cancer and the expression of TILs had significant statistical significance with TNM stage （ P = 0. 000）, tumor size （ P = 0. 002 ）, differentiation degree （ P = 0. 000 ）, invasion depth （ P = 0. 000 ）, lymph node metastasis （P =0. 000） ,perineural invasion （ P = 0. 005 ） , andtumor thrombus （ P = 0. 003 ）. TILs expression is positively related to the survival time of the patients, the median survival time was （69. 96± 2. 12）months in the patients with high expression, and the median survival time was （54.78±1.99） months in the patients with low expression （ P = 0. 000 ）. TILs [ Hazard ratio （HR） = 0. 815, P = 0. 0303, age （HR = 1. 496,P =0. 000）, TNM stage （ HR = 1. 756,P = 0. 000）, tumor thrombus （ HR = 1. 389 ,P =0. 001 ） and lymph node metastasis （HR = 1. 864 ,P =0. 000） were independent prognostic fac-tots in Cox regression model. Conclusion TILs play an important protective role in gastric cancer patients.
