少见病理亚型肾细胞癌11例动态增强CT表现及文献复习

摘　　要：目的总结几种少见病理亚型肾细胞癌的动态增强CT表现,提高影像诊断水平。方法结合相关文献,回顾性分析经手术病理证实的11例少见病理亚型肾癌患者动态增强CT表现。结果 5例乳头状细胞癌,4例平扫密度不均,1例为囊性低密度影,增强扫描呈少血供表现;4例嫌色细胞癌,3例平扫密度均匀,1例密度不均,增强扫描呈均匀或不均匀轻中度强化;1例Bellini集合管癌,平扫表现为肾脏体积增大,密度不均,肿瘤界限不清,增强扫描呈轻度不均匀延时强化,并侵犯肾周脂肪囊;1例Xp11.2易位/TFE3基因融合相关性肾癌,平扫病灶内见多发环形钙化灶,增强扫描病灶实性部分明显强化。结论 CT对少见病理亚型肾癌的定性诊断有一定的价值,但确诊还应依赖于病理学检查。Objective To summarize the dynamic contrast-enhanced CT findings of rare pathological subtypes of renal cell carcinoma,so as to improve the imaging diagnosis.Methods We retrospectively analyzed the CT manifestation of 11 patients with rare pathological subtypes of renal cell carcinoma confirmed by surgical pathology and reviewed literatures.Results In 5cases of papillary carcinoma,CT plain scan showed inhomogeneous density in 4 cases and cystic hypodense in 1 case,and enhanced CT showed insufficient blood supply.In 4 cases of chromophobe carcinoma,plain scan showed homogeneous density in 3case and inhomogeneous density in 1 case,and enhanced CT showed mild to moderate homogeneous or inhomogeneous enhancement In 1 case of Bellini collecting duct carcinoma,plain scan showed enlarged kidney volume and inhomogeneous density,ambiguous boundary;enhanced CT showed mild inhomogeneous delayed enhancement and invasion of renal adipose capsule In 1cases of Xp11.2 translocation/TFE3 fusion-associated renal carcinoma,ring-like calcifications were observed on CT scan,the enhanced CT showed that the solid portion of mass was enhanced obviously.Conclusion CT is a valuable method to diagnose rare pathological subtypes of renal cell carcinoma But the final diagnosis should be based on pathology.
