摘  要:目的探究幽门螺杆菌(Helicobacter pylori,H.pylori)感染对大肠癌的影响。方法选取2014年1月-2015年1月在河北省黄骅市人民医院重症医学科被确诊为大肠癌并接受治疗的50例患者为试验组,并选择同期通过肠镜检查、大肠黏膜表现正常的50例患者为对照组。比较两组患者H.pylori感染情况,在不同病理类型下试验组中大肠癌患者的H.pylori感染情况,并对与大肠癌有关的多种危险性因素进行非条件Logistic回归分析。结果试验组H.pylori阳性率明显高于对照组,差异具有统计学意义(P<0.05);试验组中COX-2阳性率明显高于对照组(P<0.05);试验组大肠癌患者中的黏膜内癌、腺癌及神经内分泌性肿瘤患者的H.pylori感染情况比较差异均无统计学意义(P>0.05);非条件Logistic回归分析发现,H.pylori感染是大肠癌形成的危险因素(P<0.05)。结论 H.pylori感染会增加大肠癌的发病率。Objective To investigate the effect of Helicobacter pylori( H. pylori) infection on colorectal cancer.Methods Fifty patients who were diagnosed as colorectal cancer from Jan. 2014 to Jan. 2015 in Huanghua People's Hospital were collected as the experimental group,at the same time,50 normal patients choosed by endoscopy with good colon mucosa performance were as control group. H. pylori infection in two groups,H. pylori infection in different pathological types were compared. Relevant multiple risk factors were chosen to do conditional Logistic regression analysis.Results H. pylori positive rate in experimental group was significantly higher than that in control group,the difference was statistically significant( P〈0. 05); COX-2 positive rate in experimental group was significantly higher than that in control group( P〈0. 05); there was no significant difference in H. pylori infection among mucosal cancer,adenocarcinoma,and endocrine tumors of nerve in experimental group( P〈0. 05); Logistic regression analysis indicated that H. pylori infection was a risk factor for colorectal cancer( P〈0. 05). Conclusion H. pylori infection can increase the incidence of colorectal cancer.
