摘 要:目的研究PD-L1基因多态性与肝癌易感性以及患者预后之间的关系。方法收集我院123例原发性肝癌患者和141例健康体检者的外周血淋巴细胞,应用直接测序法和PCR-SSCP检测PD-L1基因rs4143185,rs2890658和rs17718883三个位点的单核苷酸多态性。结果 rs2890658位点的AC基因型及rs17718883的CG基因型可以显著降低肝癌的发病风险(OR=0.43,P=0.035;OR=0.01,P<0.001);而rs4143185的GG基因型可能增加肝癌的发病风险(OR=2.80,P=0.057)。rs2890658位点AC+CC和AA的中位生存时间分别为20个月和17个月(P=0.042);rs4143185位点的GC+GG和CC中位生存时间分别为21个月和15个月,差异有统计学意义(P<0.001);COX回归分析显示rs4143185位点的突变基因型都可以获得好的预后,差异有统计学意义(HR<1;且P<0.05)。结论 PD-L1基因rs2890658和rs17718883两个位点的SNP与肝癌的易感性有关,而rs2890658和rs4143185位点的SNP可能与肝癌的预后有关。Objective To investigate the relationship of the single nucleotide polymorphisms (SNPs) of programmed death-ligand 1 ( PD-L1 ) gene with the susceptibility and prognoses of liver cancer. Methods Liver tissue samples from 123 patients pathologically diagnosed with primary hepatic carcinoma and 141 healthy individuals were collected in our hospital from February 2008 to May 2014. Direct sequencing and polymerase chain reaction-single strand conformation polymorphism (PCR-SSCP) were used to detect the SNPs of PD-L1 gene in 3 loci, rs4143185, rs2890658 and rs17718883. Results The heterozygous mutation of AC genotype in PD-L1 rs2890658 and CG genotype in rs17718883 reduced the risk of hepatocellular carcinoma (OR =0.43, P =0. 035; OR =0.01, P 〈 0.001 ), while the GG genotype in rs4143185 increased the risk ( OR = 2.80, P = 0. 057 ). The median survival time was 20 and 17 months respectively in the patients carrying AC + CC and AA genotypes in rs2890658 locus (P =0. 042) , and 21 and 15 months separately in those carrying GC + GG and CC genotypes at rs4143185 locus (P 〈0.01 ). Cox regression analysis suggested that the mutation genotypes of rr-M143185 resulted in good prognosis (HR 〈 1, P 〈 0.05). Conclusion PD-L1 SNPs in the loci of rs2890658 and rs17718883 are associated with susceptibility to hepatocellular carcinoma, and those of rs2890658 and rs4143185 loci are related with the prognosis.
