摘  要:目的探讨血清腺苷脱氨酶(ADA)与谷丙转氨酶(ALT)、谷草转氨酶(AST)、谷氨酰转肽酶(GGT)联合检测在肝脏疾病诊断中的价值。方法 123例肝病患者作为观察组,选择同期行健康体检的53例正常人作为对照组,均运用全自动生化分析仪联合检测两组受检者的血清ADA、ALT、AST以及GGT值,回顾性分析两组受检者的检测结果。结果观察组患者的ADA、ALT、AST以及GGT指标检测结果均显著大于对照组(P<0.05);两组受检者的ADA、ALT、AST以及GGT指标阳性率对比差异均具有统计学意义(P<0.05)。在肝病患者中,ADA阳性率最高的是肝硬化患者,占92.31%,其次是急性肝炎与肝癌患者,分别占85.48%、70.83%。急性肝炎患者的ALT与AST阳性率均为100.00%;AST阳性率在肝癌、急性肝炎、慢性肝炎、肝硬化分别占87.50%、100.00%、90.91%、80.77%;GGT阳性率最高的是肝癌患者,占75.00%,其次是急性肝炎患者,占67.74%。结论在临床诊断肝脏疾病中联合检测ADA、ALT、AST以及GGT各项指标具有显著临床价值,为临床对各类型肝脏疾病的鉴别诊断提供准确依据,值得临床大力推广。Objective To investigate value by combined detection of serum adenosine deaminase(ADA), alanine transaminase(ALT), aspartate aminotransferase(AST) and gamma-glutamyl transpeptidase(GGT) in diagnosis of hepatic disease. Methods There were 123 patients with hepatic disease as observation group and 53 healthy people at the same period as control group. They all received fully automatic biochemical analyzer for combined detection of serum ADA, ALT, AST and GGT contents. Detection outcomes in both groups were retrospectively analyzed. Results The observation group had obviously higher ADA, ALT, AST and GGT contents than the control group(P〈0.05), and the difference of positive rates of ADA, ALT, AST and GGT had statistical significance between the two groups(P〈0.05). Among patients with hepatic disease, liver cirrhosis cases had the highest positive rate of ADA, accounting for 92.31%, acute hepatitis and liver cancer cases had lower rates, accounting for 85.48% and 70.83%. Cases with acute hepatitis had positive rates of ALT and AST both as 100.00%. Positive rate of AST was respectively 87.50%, 100.00%, 90.91% and 80.77% in liver cancer, acute hepatitis, chronic hepatitis and liver cirrhosis. Cases with liver cancer had the highest positive rate of GGT, accounting for 75.00%, and cases with acute hepatitis had lower rate, accounting for 67.74%. Conclusion Combined detection of ADA, ALT, AST and GGT provides remarkable clinical value in clinical diagnosis of hepatic disease. This method provides accurate reference for differential diagnosis of various hepatic disease, and it is worth widely clinical promotion.
