外周神经电刺激引导下低浓度BTX-A多点注射治疗顽固性面肌痉挛临床观察

摘　　要：目的观察外周神经电刺激引导下低浓度A型肉毒毒素(BTX-A)局部多点注射治疗顽固性面肌痉挛的疗效和安全性。方法选择我院神经内科2015年1~6月收治的75例顽固性面肌痉挛患者,采用隐匿数字随机法分为两组,低浓度组37例给予低浓度(25 U/mL)BTX-A局部多点注射治疗,高浓度组38例给予高浓度(50 U/mL)BTX-A局部多点注射治疗。治疗前、治疗后6个月进行面肌痉挛强度评级,根据评级评价疗效,同时记录患者注射点数、起效时间、疗效持续时间和治疗次数。结果两组患者的注射点数、药物起效时间、末次注射疗效持续时间、治疗次数、治疗总有效率比较差异均无统计学意义(P>0.05);低浓度组患者的不良反应发生率为13.51%,明显低于高浓度组的44.74%,两组比较差异有统计学意义(P<0.05)。结论外周神经电刺激引导下BTX-A局部多点注射可精确定位痉挛肌注射点,低浓度可达到显著疗效,从而提高治疗的安全性。Objective To observe the efficacy and safety of low concentration botulinum toxin A（BTX-A）multi-point injection in the treatment of intractable facial spasm under the guidance of peripheral nerve electrical stimulation.Methods A total of 75 cases of intractable facial spasm,who admitted to Department of Internal Medicine-Neurology of our hospital from January 2015 to June 2015,were selected and divided into the two groups by using hidden digital random method.The low concentration group（n=37） was given low dose（25 U/mL） BTX-A local multi-point injection;the high concentration group was treated with high concentration（50 U/mL） BTX-A local multi-point injection.The curative effect was evaluated according to the facial spasm strength rating before treatment and after treatment of 6months.At the same time,the number of injection points,onset time,duration of treatment and the number of treatment were also recorded.Results There was no significant difference between the two groups in the number of injection points,drug onset time,the duration of the last injection,the number of treatment and the total effective rate（P〉0.05）.The incidence of adverse reactions of the low concentration group was 13.51%,which was significantly less than 44.74% of the high concentration group（P〉0.05）.Conclusion Peripheral nerve stimulation guided BTX-A local multi-point injection can accurately pinpoint the spasm muscle injection point,and low concentration can achieve remarkable curative effect,so as to improve the safety of the treatment.
