双镜联合切除胃间质瘤的手术效果分析

摘　　要：目的:探讨双镜联合切除胃间质瘤的手术效果及临床应用价值。方法:回顾分析2009年1月至2013年8月收治的106例胃间质瘤患者的临床资料,均择期手术治疗,其中观察组50例行双镜(腹腔镜+内镜)联合切除术,対照组56例行腹腔镜切除木,比较两组手术效果(手术时间、手术出血量、术后胃肠功能恢复时间)及术后病理检査结果。结果:观察组手术时间、术中出血量、术后胃肠功能恢复时间均显著少于对照组差异有统计学意义(P<0.05);两组下床活动时间、住院时间、术后病理结果差异均无统计学意义(P>0.05)。结论:单纯腹腔镜、双镜联合切除术治疗胃间质瘤的住院时间、术后病理结果相似,但相较单纯腹腔镜手术,双镜联合切除术可有效缩短手术时间,减少术中出血量。Objective：To investigate the operative effects and clinical application value of endoscopic and laparoscopic resection of gastric stromal tumors.Methods：The related data of 106 patients with gastric stromal tumors treated between Jan.2009 and Aug.2013 were analyzed retrospectively.All patients underwent elective operation,including 50 cases undergoing laparoscopic and endoscopic resection（observation group） and 56 cases undergoing laparoscopic resection（control group）.The operation effects（duration of operation,intraoperative blood loss,time of gastrointestinal recovery after operation） and results of postoperative pathological examination were compared between the two groups.Results：The duration of operation,intraoperative blood loss postoperative gastrointestinal recovery time of observation group were significantly less than those of the control group（P〈0.05）.There were no significant differences in ambulation time,hospital stay and postoperative pathological results（nuclear mitotic image,malignant potential grading） between the two groups（P〉0.05）.Conclusions：The long-term effects of laparoscopy alone and combined endoscopic and laparoscopic resection are similar in the treatment of gastric stromal tumors,with few complications and short hospital stay.Compared with laparoscopic surgery alone,the time of endoscopic and laparoscopic combined resection is significantly shorter,and the intraoperative blood loss is significantly less.
