CA125联合经阴道彩色多普勒超声在卵巢癌新辅助化疗监测中的应用价值

摘　　要：目的:探讨CA125联合经阴道彩色多普勒超声在卵巢癌新辅助化疗监测中的应用价值。方法:收集2013年3月至2015年3月我院妇产科收治的行新辅助化疗卵巢癌患者114例,所有入组患者均于新辅助化疗前后分别检测CA125水平及测定经阴道彩色多普勒超声相关指标,分析CA125及经阴道彩色多普勒超声指标化疗前后的差异,计算、比较二者在新辅助化疗效果监测中的敏感性、特异性及准确性。结果:新辅助化疗后CA125水平明显降低,差异有统计学意义(P<0.05),其监测敏感性为63.26%,特异性为56.25%,准确率为62.28%;新辅助化疗后,卵巢癌原发灶瘤体体积明显缩小,血流最大速度明显降低,同时血流阻力指数明显增大,差异均有统计学意义(均P<0.05),其监测敏感性为83.67%,特异性为81.25%,准确率为81.58%。CA125联合经阴道彩色多普勒超声的监测敏感性为94.89%,特异性为68.75%,准确率为91.22%,与单独应用CA125及经阴道多普勒超声监测结果具有统计学差异(P<0.05)。结论:CA125联合经阴道彩色多普勒超声可明显提高卵巢癌新辅助化疗监测的准确性,能更好的评估新辅助化疗的效果,为临床评估奠定基础。Objectives： To investigate the value of CA125 and transvaginal color Doppler ultrasonography（ TV- CD） of in monitoring the efficacy of neoadjuvant chemotherapy for ovarian cancer. Methods： 114 patients with ovarian cancer treated by neoadjuvant chemotherapy from March 2013- March 2015 in the Obstetrics and Gynecology Department of our hospital were admitted. CA125 levels and related indicators of TV- CD were detected and measured before and after neoadjuvant chemotherapy. The differences in CA125 and TV- CD before and after chemotherapy were analyzed. Sensitivity,specificity and accuracy in monitoring the efficacy of neoadjuvant chemotherapy were both calculated and compared. Results： CA125 levels after neoadjuvant chemotherapy were significantly lower than these before neoadjuvant chemotherapy,with statistically significant difference（ P 〈0. 05）,sensitivity,specificity and accuracy being 63. 26%,56. 25% and 62. 28% respectively. After neoadjuvant chemotherapy,ovarian tumor volume was significantly smaller,the maximum blood flow velocity was significantly lower,and resistance index significantly higher than them before neoadjuvant chemotherapy,all with statistically significant difference（ P〈 0. 05）,the sensitivity,specificity and accuracy being 83. 67%,81. 25% and 81. 58% respectively. In the detection of CA125 combined with TV- CD,the sensitivity,specificity and accuracy being 94. 89%,68. 75% and 91. 22% respectively with a statistically significant difference compared with CA125 and TV- CD alone（ P 〈0. 05）. Conclusion： CA125 can significantly improve the accuracy of the efficacy of neoadjuvant chemotherapy for ovarian cancer combined with TV- CD,which lays the foundation for clinical evaluation by better evaluating the effect of neoadjuvant chemotherapy.
