摘 要:目的探讨血浆多聚免疫球蛋白受体(PIgR/SC)、甲胎蛋白(AFP)对原发性肝癌(PHC)的诊断价值及联合检测的意义。方法选取50例原发性肝癌患者、30例肝良性占位患者、45例肝硬化患者和60例健康志愿者进行研究,抽取每位研究对象4 mL空腹静脉血,采用电化学发光全自动免疫分析系统检测患者血清中的甲胎蛋白(AFP)浓度,采用自行包被的ELISA检测板和OD值半定量法检测血浆PIg R/SC。肝癌患者手术切除后1周重复检测AFP和PIgR/SC水平。结果肝癌、肝良性占位、肝硬化和志愿者4组患者PIgR/SC和AFP水平差异有统计学意义(P<0.05),两两分析,肝癌患者PIg R/SC高于肝良性占位患者、肝硬化患者和志愿者(P<0.05)。PIgR/SC诊断原发性肝细胞肝癌的敏感度为76%高于AFP的66%,特异性为68.9%,低于AFP的81.5%,差异有统计学意义(P<0.05)。联合检测PIgR/SC、AFP可提高肝癌患者的诊断率。PIgR/SC与AFP串联检测诊断肝癌的特异度为95.6%,2项指标并联检测诊断肝癌的灵敏度为92.0%,明显高于单用PIgR/SC或AFP(P<0.05);结论联合检测PIgR/SC、AFP可显著提高原发性肝癌患者的诊断率。Objective To investigate the diagnostic value of plasma poly immunoglobulin receptor(PIgR/SC) and alpha fetoprotein(AFP) in primary liver cancer(PHC) and the significance of joint detection. Methods 50 cases of patients with primary liver cancer, 45 patients with liver cirrhosis,30 patients with benign liver space occupying,and 60 healthy volunteers were selected as thestudy objects.4 ml fasting venous blood was collected from each research object. Serum level of AFP was detected with electrochemical chemiluminescence immunoassay system, and plasma PIgR/SC level was detected by ELISA method. The levels of AFP and PIgR/SC were detected in 1 weeks after hepatectomy. Results The levels of AFP and PIgR/SC in the four groups were significantly different(P〈0.05), and PIgR/SC was higher than that in patients with cirrhosis, benign liver space occupying and volunteers(P〈0.05). Sensitivities of PIgR/SC and AFP were 76% and 66%, specificities of PIgR/SC and AFP were 68.9% and 81.5%. The difference was statistically significant(P〈0.05). The specificity of PIgR/sc and AFP tandem detection in diagnosing PHC was 95.6%, and the sensitivity of these two markers for parallel detection was 92%. It was significantly higher than single PIgR/sc or AFP(P〈0.05). Conclusion Joint detection of PIgR/SC and AFP can significantly improve the diagnosisrate rate of PHC.
