人附睾蛋白4与卵巢癌疾病转归的关系探讨

摘　　要：目的探讨人附睾蛋白4（HE4）水平在卵巢癌疾病转归中的价值。方法收集盆腔肿物患者112例,其中卵巢上皮性恶性肿瘤94例,卵巢非上皮性恶性肿瘤18例。应用电化学发光法测定患者手术前后血清HE4和糖类抗原125（CA125）的水平,并随访其中经过满意的全面分期手术的36例上皮性肿瘤患者术后血清HE4和CA125的水平,分析HE4水平与肿瘤复发的关系。结果患者术后血清HE4水平为32.19pmol/L（21.67-293.40pmol/L）,明显低于术前水平63.37pmol/L（29.10-444.80pmol/L）,二者差别有统计学意义（Z=-2.310,P=0.021）。随着化疗后临床缓解,患者的HE4水平下降;当患者病情复发或盆腔出现病灶时,血清HE4水平再度升高。36例上皮性卵巢癌患者经15月随访后,复发5例,盆腔MRI检查示肿瘤直径约4-6cm,血清HE4水平均明显升高,且血清HE4的升高比CA125提前4-12周。结论 HE4水平与卵巢癌的临床转归密切相关,是监测患者病情的敏感肿瘤标志物。Objective To investigate the value of human epididymis protein 4（HE4）in the prognosis of ovarian cancer disease. Method The levels of HE4 and carbohydrate antigen-125（CA125）were detected by using the chemiluminescence method in 94 patients with epithelial ovarian and 18 patients with non epithelial tumor before and after surgery. The levels of serum HE4 and CA125were followed for 36 patients with epithelial ovarian tumor after operation and the relationship between HE4 level and tumor recurrence was analyzed. Result In 112 patients with ovarian cancer after a satisfactory surgical staging,the postoperative serum level of HE4 was 32.19pmol/L（21.67-293.40pmol/L）and was significantly lower than the preoperative serum levels：63.37pmol/L（29.10-444.80pmol/L）,the difference was statistically significant（Z=-2.310,P=0.021）. Moreover,while the patients were in clinical remission after chemotherapy,the level of HE4 was decreasing and the level of serum HE4 was elevated in the patients with recurrent disease or pelvic lesions. In 36 cases after satisfactory comprehensive surgical staging of epithelial ovarian cancer the patients were followed up for 15 months,5of them had disease recurrence and pelvic MRI found tumors with a diameter of about 4-6cm,serum HE4 levels were significantly increased. The serum HE4 increased 4-12weeks earlier than CA125. Conclusion The level of HE4 is closely related to clinical outcome of ovarian cancer,and is a sensitive tumor marker for disease monitoring
