遗忘型与非遗忘型轻度认知功能障碍的精神行为症状比较

摘　　要：目的探讨遗忘型与非遗忘型轻度认知功能障碍(MCI)的精神行为症状的特征及差异。方法选取2012年1月至2016年3月于我院门诊和住院的遗忘型MCI患者63例(A组)和非遗忘型MCI患者54例(B组)作为研究对象,采用神经精神问卷对两组患者精神行为症状特点进行评估。结果 A组的MMSE评分显著低于B组(P<0.05),A组的ADAS-COG评分显著高于B组(P<0.05),记忆因子评分A组显著高于B组,差异具有统计学意义(t=4.730,P<0.001),语言因子评分比较A组显著高于B组,差异具有统计学意义(t=3.126,P<0.001),注意力因子比较A组显著高于B组(P<0.05),其中A组患者的睡眠行为异常和食欲障碍比例显著高于B组(P<0.05),B组患者的激越、淡漠和脱抑制比例显著高于A组,差异具有统计学意义(P<0.05)。结论非遗忘型MCI患者的认知功能优于遗忘型MCI患者,遗忘型MCI患者的睡眠行为异常和食欲障碍比例更高,而非遗忘MCI患者的激越、淡漠和脱抑制比例显著高于遗忘MCI患者。Objective To study the characteristics and differences in behavioral and psychological symptoms between amnestic type and non-amnestic type mild cognitive impairment patients. Methods 63 amnestic type mild cognitive impairment patients and 54 non-amnestic type mild cognitive impairment patients were selected from our hos- pital since January 2012 to March 2016, using the neural mental questionnaire to evaluate the mental behavior charac- teristics of patients in two groups. Results The MMSE score of patients in group A was significantly lower than that in group B （P 〈 0.05 ）, ADAS - COG score of patients in group A was significantly higher than that in group B （P〈 0.05 ） . The memory factor score of patients in group A was significantly higher than that of group B （ t = 4.730, P〈0.001 ） . The language factor score of patients in group A was significantly higher than that of group B （ t = 3.126, P〈0.001 ） . The concentration factor of patients in group A is significantly higher than that of group B （ P〈0.05 ） . The patients in group A had a higher abnormal sleep behavior and appetite rate than that in group B （P 〈 0.05 ） . The agitation, indif- ference and inhibition ratio of patients in group B was significantly higher than those in group A （P 〈 0.05 ） . Conclusion The cognitive function of the non-amnestic type mild patients were superior to that in the amnestic type mild cognitive impairment patients, the rate of abnormal sleep behavior and appetite in amnestic type MCI patients had higher than those in the amnestic type MCI patients. However, the agitation, indifference and depression rate in non-amnestic type MCI was significantly higher than those in amnestic type MCI patients.
