缺血缺氧性脑病新生儿血清 IL-1β、IL-6和 TNF -α水平变化及其临床意义

摘　　要：目的：探讨缺血缺氧性脑病（HIE）新生儿血清白细胞介素-1β（ IL-1β）、IL-6和肿瘤坏死因子-α（TNF-α）水平变化及其临床意义。方法对确诊的60例 HIE 患儿根据病情分轻度组（25例）、中度组（20例）和重度组（15例），并选择同期正常分娩新生儿30例作健康对照，ELISA 法检测血清中 IL-1β、IL-6和 TNF-α的表达水平。结果 HIE 新生儿急性期血清 IL-1β、IL-6和 TNF-α表达水平均显著高于对照组（P 〈0.01），恢复期明显下降（P〈0.01）。不同病情程度 IL-1β、IL-6和 TNF-α表达差异有统计学意义（P 〈0.05）。患儿血清 IL-1β、IL-6和 TNF-α表达呈正相关（r =0.581、0.568、0.512，P 〈0.01），并与患儿的病情程度呈正相关（r =0.936、0.935、0.769，P 〈0.01）。急性期血清 IL-1β、IL-6和 TNF-α的 ROC 曲线下面积分别为0.948、0.982、0.913（P 〈0.01）。结论 IL-1β、IL-6和 TNF-α在新生儿 HIE 的发病过程中发挥着重要作用；其在血清的表达水平具有较高的诊断价值，可作为新生儿 HIE 诊断和病情监测的临床指标。Objective To investigate the variation and clinical significance of serum IL-1β,IL-6 and TNF-α in the neonatal hypoxic-ischemic encephalopathy（HIE）. Methods Sixty patients with HIE were divided into mild group（25 cases）,moderate group（20 cases）and severe group（15 cases）ac-cording to the condition,and 30 cases of normal newborns at the same period were enrolled into health control group,the expression levels of IL-1β,IL-6 and TNF-α in serum were detected by ELISA. Results The expression levels of serum IL-1β,IL-6 and TNF-α in the acute phase of HIE newborns were significantly higher than those of normal control group（P 〈 0. 01）,and decreased significantly in recovery period（P 〈 0. 01）. The expression level of serum IL-1β,IL-6 and TNF-α had significant differ-ences in different disease degree（P 〈 0. 05）. The levels of IL-1β,IL-6 and TNF-α were significant pos-itive correlation（r = 0. 581,0. 568,0. 512,P 〈 0. 01）;and they were positively correlated with the de-gree of children with the disease（r = 0. 936,0. 935,0. 769,P 〈 0. 01）. The area under the ROC curve of serum IL-1β,IL-6 and TNF-α in patients with acute phase serum were 0. 948,0. 982,0. 913 respec-tively（P 〈 0. 01）. Conclusions IL-1β,IL-6 and TNF-α play an important role in the pathogenesis of neonatal HIE process and exists the interaction;the expression levels in serum have high diagnostic val-ue,which can be used as indicators of clinical diagnosis and disease monitoring for neonatal HIE.
