摘 要:目的:探讨胃癌患者人表皮生长因子受体2(HER-2)基因扩增及蛋白表达与临床病理特征的关系。方法对98例进展期胃癌患者采用荧光原位杂交(FISH)方法进行HER-2基因检测,采用免疫组织化学(IHC)方法进行HER-2蛋白检测,分析HER-2基因扩增及蛋白表达与胃癌患者临床病理特征的相关性。结果98例胃癌标本中,16例(16.3%)HER-2基因扩增,82例(83.7%)无扩增;31例(31.6%)HER-2蛋白表达阴性,58例(59.2%)HER-2蛋白表达不确定,9例(9.2%)HER-2蛋白表达阳性。胃癌组织不同强度HER-2蛋白表达与基因扩增结果符合率分别为96.7%(30/31)、12.0%(7/58)、88.9%(8/9), HER-2蛋白表达与其基因扩增有相关性(P<0.001)。 HER-2基因扩增与患者性别、年龄、肿瘤分化程度、浸润深度无相关性(均P〉0.05),与淋巴结转移有关(P=0.022)。结论胃癌患者HER-2蛋白表达与基因扩增结果有较高符合率,IHC检测HER-2蛋白表达不确定时和基因检测结果有较大差异,建议进一步采用FISH检测HER-2基因。胃癌患者HER-2基因扩增与淋巴结转移有关,可为判断胃癌生物学行为提供参考。Objective To investigate the relationship between human epidermal growth factor receptor 2 (HER-2) gene amplification and protein expression in gastric carcinoma and its clinicopathological features. Methods Gene expression and protein expression of 98 gastric cancer patients were detected by fluorescence in situ hybridization (FISH), and immunohistochemistry (IHC) staining respectively. The relationship between gene amplification, protein expression of HER-2 and the clinicopathological characteristics were analyzed. Results In 98 cases of gastric cancer, there were 16.3 % (16/98) with gene amplification of HER-2, 83.7 % (82/98) without gene amplification of HER-2; 31.6 % (31/98) with negative expression of HER-2 protein, 59.2 % (58/98) with uncertain expression of HER-2 protein, 9.2 % (9/98) with positive expression of HER-2 protein. The coincidence rates of HER-2 protein expression and gene amplification were 96.7 % (30/31), 12.0 % (7/58), 88.9 % (8/9). The expression of HER-2 protein and gene amplification in gastric cancer was significantly correlated (P〈 0.001). HER-2 gene amplification was not significantly correlated with gender, age, differentiation degree and depth of invasion (all P〉 0.05), but with lymph node metastasis (P= 0.022). Conclusions The negative and positive expression of HER-2 protein have a high coincidence rate with gene amplification in gastric carcinoma. However, the results of IHC and FISH are different when the expression of HER-2 protein is uncertain. The amplification of HER-2 gene is associated with lymph node metastasis in patients with gastric cancer, which may provide an important reference index for determining the biological behavior of gastric cancer.
