不同解剖段胃肠间质瘤病人术后生存比较分析

摘　　要：目的:比较不同解剖位置胃肠道间质瘤切除术后病人的生存差异。方法:选取1999年1月至2010年12月美国流行病学统计资料(SEER)患有胃肠间质瘤并行手术治疗的病人3 593例,排除行放射治疗、术后生存期不足1个月且非间质瘤病因死亡病人462例,3 131例纳入本研究。采用Kaplan-Meier生存曲线分析间质瘤不同部位和生存率的关系。结果:胃、小肠、结肠和直肠间质瘤病人分别占58.3%、35.7%、3.4%和2.7%。胃与小肠间质瘤5年生存率相似(83.5%比81.3%),高于结肠间质瘤(65.9%),直肠间质瘤最高(92.4%,P<0.001)。随间质瘤直径增加和分化程度降低,病人生存率降低。结论:不同部位的胃肠道间质瘤病人术后生存时间存在明显差异,以结肠间质瘤预后最差。Objective To explore the differences in survival of the patients after surgical resection of gastrointestinal stromal tumor（GIST） of stomach, intestine, colon and rectum. Methods A total of 3 593 patients undergoing GIST resection from January 1999 to December 2010 were identified from Surveillance, Epidemiology, and End Results database.Exclude were the patients who underwent radiotherapy, survival less than one month after surgical resection and who die due to other reasons. There were 3 131 patients enrolled in this study. Kaplan-Meier survival curve was used to analyze the survival of GIST of stomach, intestine, colon and rectum. Results The patients with GIST of stomach, intestine, colon and rectum were 58.3%, 35.7%, 3.4% and 2.7%, respectively. Five-year survival of the patients with gastric GIST（83.5%）was similar to that with intestinal GIST（81.3%） and longer than that with colon GIST（65.9%）. The longest 5-year survival was 92.4% in the patients with rectum GIST（P〈0.001）. It was found that the survival rate of the patients decreased when both the diameter of GIST increased and differentiation extent was lower. Conclusions There was much difference in the survival time of the patients with GIST of stomach, intestine, colon or rectum after resection. The patients with GIST of colon have the poorest prognosis in the patients with GIST.
