尿激酶溶栓联合阿司匹林治疗风湿性心脏病左心房内血栓脱落致脑栓塞的疗效

摘　　要：目的分析尿激酶溶栓联合阿司匹林在风湿性心脏病左心房内血栓脱落所致脑栓塞治疗中的应用价值。方法应用随机数字表法,将60例风湿性心脏病左心房内血栓脱落所致脑栓塞患者分为2组,每组30例。对照组接受包括阿司匹林在内的常规治疗,观察组接受联合尿激酶溶栓及阿司匹林治疗,对比2组治疗效果。结果对照组治疗前后NIHSS评分及ADL评分无明显变化(P>0.05),观察组治疗前后NIHSS评分呈下降趋势,ADL评分呈升高趋势(P<0.05);2组治疗前NIHSS评分及ADL评分比较,差异无统计学意义(P>0.05),而观察组治疗1d、2d及3d时NIHSS评分低于对照组,ADL评分高于对照组(P<0.05)。2组不良反应发生率比较,差异无统计学意义(P>0.05)。结论联合尿激酶溶栓及阿司匹林可有效改善风湿性心脏病左心房内血栓脱落致脑栓塞的疗效。Objective To discuss the application value of urokinase thrombolysis combined with aspirin in the treatment of cerebral embolism caused by the fall-off of embolus from left atrium in patients with rheumatic heart diseases. Methods Six ty cases conforming to inclusion criteria were divided into two groups,30 cases in each group. Control group received conven- tional treatment including aspirin,while observation group received urokinase thrombolysis combined with aspirin. Therapeutic effects of the two groups were compared. Results There were no obvious changes in the terms of NIHSS scores and ADI. scores before and after treatment in the control group（P〉0.05）, but the observation group showed decreasing trend of NIHSS scores and increasing trend of ADL scores after treatment（P〈0.05）. At the same time, no significant differences involving to NIHSS scores and ADL scores were found between the two groups before treatment（P〉0.05）, however, on the first, second and third day after treatment,the observation group got less NIHSS scores and more ADL scores than the control group（P〈 0.05）. The incidence of adverse events showed no statistical difference between the two groups（P〉0.05）. Conclusion The combination of urokinase thrombolysis and aspirin can effectively improve the clinical effects on cerebral embolism caused by the fall-off of embolus from left atrium in patients with rheumatic heart diseases.
