人乳头瘤病毒E6/E7 mRNA在宫颈疾病筛查中的临床价值

摘　　要：目的探讨人乳头状瘤病毒(human papillomavirus,HPV)E6/E7 mRNA定量检测在宫颈病变筛查中的价值。方法同时对217例我院就诊患者行液基薄层细胞学检测(thin-prep cytologic test,TCT)和HPV E6/E7 mRNA定量检测。结果 HPV E6/E7 mRNA阳性率随着细胞学和病理学宫颈病变级别升高呈趋势性增加。HPV E6/E7 mRNA对子宫颈上皮内瘤变(cervical intraepithelial neoplasia,CIN)2级以上(CIN2+)诊断的灵敏度高于TCT,特异度低于TCT,差异有统计学意义(P<0.05)。HPV E6/E7 mRNA和TCT联合平行(并联)试验筛查效果较好,灵敏度和特异度分别为87.8%[95%CI(73.8%,95.9%)]和79.0%[95%CI(72.2%,84.8%)]。结论 TCT和HPV E6/E7 mRNA联合检测能够提高对宫颈病变筛查的敏感性和特异性,对防治宫颈病变有较好的临床价值。Objective To investigate the value of quantitative detection of Human Papillomavirus（HPV） E6/E7 mRNA in cervical lesion screening.Methods Thinprep Cytologic Test（TCT） and HPV E6/E7 mRNA quantitative determination were adopted in the same time for 217 cases of patients in our hospital.Results The positive rate of HPV E6/E7 mRNA rose following the elevation of cytology and pathology grade of cervical lesions.The diagnostic sensitivity of HPV E6/E7 mRNA on cervical intraepithelial neoplasia（CIN） above grade 2（CIN2＋） was higher than TCT,while its specificity was lower than TCT,with statistically significant differences（P〈0. 05）.Combined parallel test of HPV E6/E7 mRNA and TCT had good screening effect,the sensitivity and specificity of which were 87. 8%[95%CI（73. 8%,95. 9%） ]and 79. 0%[95%CI（72. 2%,84. 8%） ],respectively.Conclusion The combined detection of TCT and HPV E6/E7 mRNA can improve the sensitivity and specificity of screening for cervical lesions,and has a good clinical value for the prevention and treatment of cervical lesions.
