超声引导下经腋静脉和颈内静脉置管在颅脑肿瘤围术期患者中的应用对比研究

摘　　要：目的比较超声引导下经腋静脉和颈内静脉置管在颅脑肿瘤围术期患者中的应用效果。方法选择我院261例术前1d接受中心静脉置管的颅脑肿瘤患者为研究对象,将64例采用超声引导下经腋静脉穿刺置管的患者为观察组,197例采用超声引导下经颈内静脉穿刺置管的患者为对照组,观察两组患者置管并发症、导管留置时间以及患者舒适度。结果观察组置管并发症低于对照组,导管留置时间、患者舒适度均优于对照组,差异有统计学意义(P<0.05)。结论对于颅脑肿瘤围术期患者,经腋静脉穿刺置管优于颈内静脉置管,并且对常用的中心静脉置管存在禁忌症时可提供新的路径,值得临床推广。Objective To compare the effect of ultrasound guided axillary vein and internal jugular vein catheter in patients with brain tumor during perioperative period.Methods 261 cases of brain tumor patients who received central venous catheter 1d before operation in our hospital were selected as study subjects.64 cases treated by ultrasound guided axillary vein catheterization as the observation group,while 197 cases treated with internal jugular vein catheterization under ultrasound guidance as the control group.The complications of indwelling catheter,the catheter indwelling time,and patient comfort were observed in the two groups.Results The complications of observation group were lower than that of control group（P〈0. 05）,the catheter indwelling time and comfort of patients of observation group were both better than those of control group（P〈0. 05）.Conclusion For perioperative patients with brain tumors,the axillary vein catheterization is better than internal jugular vein catheter,and it can provide a new path for contraindications of common central venous catheter,which is worthy of clinical application.
