自锁托槽矫治技术在成人正畸治疗中的应用及对牙周状况和生活质量的影响

摘　　要：目的:探讨自锁托槽矫治技术在成人正畸治疗中的应用及对牙周状况和口腔健康相关生活质量(OHRQOL)的影响。方法:选取笔者医院需进行牙齿正畸的患者94例,并按照矫治技术的不同分为治疗组和对照组。对照组:47例,采用传统托槽矫治技术;治疗组:47例,采用自锁托槽矫治技术进行治疗。观察并记录两组患者治疗前后牙周情况及OHRQOL,检测并比较两组患者治疗前、后龈沟液中炎症因子水平。结果:治疗后两组PD、CAL、SBI和PLI均显著低于治疗前,GR显著高于治疗前(P<0.05或P<0.01),且两组间差异显著(P<0.05或P<0.01);治疗后两组生理性疼痛、功能受限、社交困难、生理障碍、心理沟通、心理阻碍和残障评分均显著高于治疗前(P<0.01),且治疗组显著高于对照组(P<0.01);治疗后两组龈沟液中IL-1β和TNF-α水平均显著低于治疗前,且治疗组显著低于对照组(P<0.05或P<0.01)。结论:成人正畸治疗中采用自锁托槽矫治技术可有效改善患者牙周情况和口腔健康,降低牙周环境中炎症因子水平,效果显著,值得临床推广应用。Objective To investigate the application of self-locking bracket orthodontic technique in adult orthodontic treatment and its effect on periodontal status and oral health-related quality of life（OHRQOL） effect. Methods 94 cases of dental orthodontic patients were selected, all patients were divided into treatment group（n=47） and control group（n=47） according to the different treatment techniques. The control group used the traditional bracket technique, the treatment group were treated with self-locking bracket orthodontic technique. The periodontal condition and OHRQOL scores were observed and recorded before and after treatment in 2 groups. The levels of inflammatory factors in gingival crevicular fluid were detected and compared between the 2 groups before and after treatment. Results The PD, CAL, SBI and PLI of 2 groups were significantly lower after treatment than before treatment, GR was significantly higher than before treatment（P〈 0.05 or P〈 0.01）, and the difference between the 2 groups was significant（P〈 0.05 or P〈 0.01）; The physiological pain, limited function, social difficulties, physical barriers, psychological communication and psychological obstacles and disability scores of 2 groups were significantly higher after treatment than those before treatment（P〈 0.01）, and the treatment group was significantly higher than the control group（P〈0.01）; After treatment, IL-1β and TNF-α in gingival crevicular fluid of 2 groups were significantly lower than before treatment, and the treatment group was significantly lower than the control group（P〈 0.05 or P〈 0.01）. Conclusion The self-locking bracket orthodontic technique can effectively improve the periodontal status and oral health in the adult orthodontic treatment, reduce the levels of inflammatory factors, have significant effect, be worthy of clinical application.
