龈下菌斑对慢性牙周炎患者种植体周围边缘骨吸收的影响

摘　　要：目的:分析探讨龈下菌斑对慢性牙周炎患者种植体周围边缘骨吸收的影响。方法:选取慢性牙周炎牙列缺损及牙周健康牙列缺损患者进行种植手术,观察两组在术前术后改良菌斑指数、改良龈沟出血指数和种植体周骨吸收情况并进行统计分析。结果:种植前观察组的龈下菌斑明显高于对照组(P<0.05)。经过种植术后,两组患者组内比较,龈下菌斑均明显减少,差异有统计学意义(P<0.05),而两组之间龈下菌斑对比差异无统计学意义(P>0.05)。术前两组患者改良龈沟出血指数(modified sulcus bleeding index,m SBI)对比差异无统计学意义(P>0.05),而经过种植术后,两组患者组内比较m SBI均明显低于术前(P<0.05),组间比较发现观察组术后m SBI明显高于对照组(P<0.05)。两组患者术前边缘骨吸收比较差异无统计学意义(P>0.05),而术后两组种植体周围边缘骨吸收较术前均明显减少(P<0.05)。术后观察组近中和远中边缘骨吸收量明显降低,其中近中降低较对照组更明显(P<0.05)。结论:慢性牙周炎患者的龈下菌斑多于牙周健康者且影响种植体边缘骨的吸收,进行牙周基础治疗有助于种植体周围边缘骨的稳定。Objective：To investigate the influence of gingival plaque on the absorption of bone around the implants in patients with chronic periodontitis.Method：Chronic periodontitis dentition defect and periodontal health of dentition defect patients with implant surgery were selected.The two groups of modified plaque index before and after surgery,modified sulcus bleeding index and bone resorption around the implants were observed and statistically analyzed.Result：Before implantation,the plaque of the observed group was significantly higher than that of the control group（P〈0.05）.After implantation,the plaque of the two groups was significantly reduced,the difference was statistically significant（P〈0.05）,and there was no significant difference between the two groups（P〈0.05）.Two groups of patients with preoperative modified sulcus bleeding index（modified sulcus bleeding index, m SBI） had no significant difference（P〈0.05）,but after implantation,the m SBI of the two groups was significantly lower than that before operation（P〈0.05）,the m SBI of the observation group was significantly higher than the control group（P〈0.05）.Before implantation,the marginal bone loss of the two groups had no statistical difference（P〈0.05）,but after implantation,the marginal bone loss of the two groups reduced significantly（P〈0.05）.The edge of the bone absorption in the near and far in the observation group reduced significantly,the reduce of the near in the observation group was more significant（P〈0.05）.Conclusion：In patients with chronic periodontitis,the gingival plaque is more than periodontal health patients,and it affects the implant marginal bone absorption,and the periodontal foundation treatment is helpful to the stability of the peripheral edge of the implant.
