摘  要:目的探讨血清甲状腺球蛋白(thyroglobulin,Tg)检测联合SPECT/CT融合影像技术在甲状腺癌(thyroidcancer,TC)诊断中的临床价值。方法随机选取本院2012年3月至2016年4月收治的165例分化型甲状腺癌(differentiated thyroid cancer,DTC)全切术后的患者为研究对象。^131I治疗前检查血清Tg,治疗一周后进行全身扫描,SPECT/CT融合显像检查。通过结合淋巴结穿刺活检实验以及临床随访来最终确定颈部淋巴结是否发生转移。结果确诊有80例患者发生淋巴转移。SPECT/CT影像检查的灵敏度高于血清Tg(χ^2=17.32,P〈0.05),二者联合联合诊断,可提高敏感度并显著高于单独SPECT/CT检测(χ^2=5.71,P〈0.05)。结论在DTC患者术后淋巴转移中,SPECT/CT检测与血清Tg结合,有助于提高DTC淋巴结转移的诊断灵敏度,值得临床广泛推广。Objective To investigate the clinical value of serum thyroglobulin (Tg) detection combined with SPECT/CT fusion imaging in the diagnosis of metastatic thyroid carcinoma. Methods 165 patients with differentiated thyroid cancer (DTC) undergoing total thyroidectomy in our hospital during Mar. 2012 to Apr. 2016 received the examination of serum Tg before the treatment with ^131I. After a week's treatment, they received the whole body scan and SPECT/CT fusion imaging detection. Through the combination of lymph node biopsy and clinical follow-up, determined whether the cervical lymph node metastasis. Results Lymph node metastasis occurred in 80 patients. The sensitivity of SPECT/CT detection was significantly higher than that of serum Tg (χ^2=17.32, P〈0.05), the sensitivity of combined diagnosis was significantly higher than that of SPECT/CT detection (χ^2=5.71, P〈0.05). Conclusion In lymph node metastasis of DTC patients, SPECT/CT detection combined with serum Tg detection can help to improve the diagnostic sensitivity of DTC lymph node metastasis, which is worth to be widely used in clinical.
