摘  要:目的观察IgA肾病(IgAN)大鼠肾组织中T细胞免疫球蛋白与黏蛋白域基因-1(TIM-1)、γ干扰素(IFN-γ)、白细胞介素-4(IL-4)的表达,探讨TIM-1和IFN-γ、IL-4及IFN-γ/IL-4的关系及其在IgAN大鼠发病中的作用。方法 20只雄性SD大鼠随机分成IgAN模型组和对照组,每组10只。在造模完成后处死大鼠,取肾脏,肾组织切片行PAS染色、免疫荧光观察大鼠肾脏病理变化,并行免疫组化法检测肾组织中TIM-1、IFN-γ、IL-4的表达。结果成功建立IgAN动物模型。第9周末IgAN模型组24 h尿蛋白定量显著高于对照组,血浆Alb显著低于对照组(均P<0.01)。免疫荧光显示,IgAN模型组大鼠系膜区可见团块状或颗粒状绿色IgA荧光。PAS染色的肾小球内可见明显的系膜细胞增多,系膜基质中到重度增生,肾间质内可见明显的炎性细胞浸润。Katafuchi评分IgAN模型组明显高于对照组(P<0.01)。IgAN模型组肾组织中TIM-1的表达量为7.50±1.58,较对照组(1.02±0.82)显著增加(P<0.01)。IgAN模型组IFN-γ的表达量为2.63±0.97,与对照组(2.82±1.03)比较,差异无统计学意义(P>0.01)。IgAN组IL-4的表达量为7.54±1.58,较对照组(3.43±1.35)显著增加,差异有统计学意义(P<0.01)。TIM-1表达量与IL-4呈正相关(r=0.99,P<0.01);与IFN-γ/IL-4呈负相关(r=-0.751,P<0.01)。结论 TIM-1可能通过调控Th免疫而参与IgAN的发生发展。Objective To investigate the expression of T- cell immunoglobulin domain and mucin domain- 1( TIM- 1),interferon- γ( IFN- γ),and interleukin- 4( IL- 4) in renal tissues of IgA nephropathy rats,and to explore the correlation between TIM- 1 and the imbalance of IFN- γ / IL- 4 and its role in the pathogenesis of IgA nephropathy. Methods Twenty male SD rats were randomized into 2 groups,the model group( n = 10) and the control group( n = 10). The pathological changes of kidney were assessed after PAS staining and immunofluorescence. Immunohistochemical staining was used to monitor the expression of TIM- 1,IFN- γ and IL- 4. Results The IgA nephropathy rat model was successfully established. At the ninth weekend,24- hour urine protein quantitation in the model group was significantly higher than that in control group( P〈0. 01),with significantly lower serum albumin( P〈0. 01). Immunofluorescence showed that IgA fluorescence was observed in the mesangial area of model group. PAS staining showed that the number of mesangial cells was obviously increased in glomerular of model group,with moderate to severe hyperplasia of mesangial matrix and obvious inflammatory cell infiltration in renal interstium. Katafunchi scoring in model group was significantly higher than that in control group( P〈0. 01). The expression quantity of TIM- 1 in renal tissue of model group( 7. 50 ± 1. 58) was significantly higher than that in control group( 1. 02 ± 0. 82,P〈0. 01). There was no significant difference in expression of IFN- γ between model group and control group( 2. 63 ± 0. 97 vs. 2. 82 ± 1. 03,P〈0. 01).The expression of IL- 4 in model group was significantly higher than that in control group( 7. 54 ± 1. 58 vs. 3. 43 ± 1. 35,P〈0. 01). The expression of TIM- 1 was significantly positively correlated with IL- 4( r = 0. 99,P〈0. 01),but significantly negatively correlated with IFN- γ / IL- 4( r =- 0. 751,P〈0. 01).
