有晶状体眼后房型人工晶状体植入术矫正高度近视的临床观察

摘　　要：目的:观察有晶状体眼后房型人工晶状体(PPC-IOL)植入治疗高度近视的有效性、稳定性及安全性。方法:对98例152眼行PPC-IOL术的高度近视患者进行为期12个月的随访,有效随访72例138眼。术后6个月、12个月时检测裸眼远近视力(UCVA)、最佳矫正视力(BCVA)、等效球镜屈光度(SE),测量眼压、角膜内皮细胞计数、中央前房深度(ACD)、PPC-IOL距晶状体中央前表面的距离等。结果:术后各时间段UCVA、BCVA均比术前提高,差异均有统计学意义(P<0.001)。术后各时间段SE均较术前显著降低,差异有统计学意义(P<0.001),而且术后角膜内皮细胞、眼压、PPC-IOL距原晶状体间的距离均较术前无明显改变(P>0.05),术后各时段与术前相比,ACD变浅,差异有统计学意义(P<0.05),无影响视力的严重并发症。结论:PPC-IOL术矫正高度近视安全有效,术后视力改善显著,无严重并发症发生。Objective：To observe the effectiveness,stability and safety of Effectiveness,stability and safety. Methods：All98 patients（152 eyes） with high myopia who underwent PPC- IOL operation were followed up for 12 months. Effective follow- up of 72 cases（138 eyes）. The uncorrected visual acuity（ UCVA）,best corrected visual acuity（ BCVA）,equivalent spherical refraction（ SE）,intraocular pressure,corneal endothelial cell count,central anterior chamber depth（ ACD）,PPC- IOL distance from the central front surface of the lens were measured at 6 months and 12 months postoperatively. Results：The UCVA and BCVA 6 and 12 months after treatment were significantly higher than that before treatment（ all P〈0. 001）. Further more,the spherical equivalent（ SE） either time after operation was markedly lower than that before treatment（ all P〈0. 001）.The corneal endothelial cells,intraocular pressure and the distance between the PPC- IOL and crystalline lens three times point after treatment has no obvious change compared with that before treatment（ all P〈0. 05）. There was a significant difference in ACD at any time frame of after treatment compared to before treatment（ P〈0. 05）. Conclusion：It was safe and effective by transplantation PPC – IOL in the correction of high myopia.
