摘 要:目的探讨血清Th1、Th2亚群细胞因子在眼球穿通伤患者治疗前后变化的临床意义。方法眼球穿通伤组30例(30只眼),并设立健康人对照组30例(30只眼)。采用ELISA法检测血清细胞因子IL-2、IFN-γ、IL-6、IL-10含量的动态变化。结果 (1)治疗组受伤早期较对照组除IFN-γ降低外,其余均升高,其中IL-2、IL-6、IL-10升高明显,差异有统计学意义(P<0.05);(2)治疗组治疗前后检测的4种细胞因子水平均降低,其中IL-2、IL-6、IL-10降低明显,差异有统计学意义(P<0.05);(3)治疗组入院治疗后的白细胞(WBC)和中性粒细胞(N)明显降低,视力改善水平升高,差异均有统计学意义(P<0.05)。结论眼球穿通伤患者治疗前后血清中Th1、Th2亚群细胞因子的动态变化与疾病发生及病程进展有密切联系,并且经抗感染治疗后可降低Th1、Th2炎性因子的大量分泌和WBC、N的增殖,有利于降低自身免疫性交感性眼炎的发生。Objective To investigate the changes in serum Th1 and Th2 subgroup cytokine levels after ocular perforating injury. Methods Subjects included 30 patients( n=30 eyes) with eye perforating injury and 30 healthy persons. The dynamic changes in serum cytokines IL-2,IFN-γ,IL-6,IL-10 were detected by ELISA. Results 1) Except IFN-γ,serum levels of IL-2,IL-6,IL-10 all increased significantly soon after injury( P〈0.05).2) After treatment,serum levels of all 4 cytokines,particularly IL-2,IL-6 and IL-10,decreased. 3) total WBC and neutrophil also significantly decreased. Visual acuity improved significantly. Conclusions The dynamic changes of Th1 and Th2 subgroup cytokines in perforating injury is closely related to disease progression. Appropriate treatment could decrease the production of Th1 and Th2 derived inflammatory factors,proliferation of white blood cells and neutrophil,and therefore prevent further eye inflammation and reduce the risk of sympathetic ophthalmia.
