白血病和非白血病骨髓细胞WT1基因表达水平及对预后的影响

摘　　要：目的研究白血病和非白血病患者骨髓细胞WT1基因表达水平及对预后的影响。方法随机选取2012~2015年270例血液肿瘤患者,利用PCR分析计算所有患者的WT1基因拷贝数/内参基因拷贝数(NQ比值),以此作为WT1基因的相对表达水平,并比较白血病患者180例和非白血病患者90例的WT1基因表达情况。结果所有患者经治疗后骨髓细胞WT1基因表达水平都明显降低,其中以治疗后的白血病患者骨髓细胞WT1基因表达水平降低最为明显;治疗前,白血病患者组骨髓细胞WT1基因表达水平均高于非白血病患者组,差异具有统计学意义(P<0.05);治疗后,白血病患者组骨髓细胞WT1基因表达水平和非白血病组相当低,差异无统计学意义(P>0.05)。结论骨髓细胞WT1基因高表达是对白血病患者的预判指标之一;骨髓细胞WT1基因的低表达是判断白血病患者预后是否良好的指标;骨髓细胞WT1基因低表达不排除患者有多发性骨髓瘤或淋巴瘤的可能性,临床上需进一步检查。Objective To study WT1 gene expression in bone marrow cells of patients with leukemia and non leukemia and its influence on prognosis. Methods 270 patients diagnosed as blood tumor were selected in Hematology Department of our hospitall during 2012 to 2015 by the method of random selection. And then the WT1 gene copy number / gene copy number（ the ratio of NQ gene copy） with PCR were calculated and analyzed,and the gene expression in 180 leukemia patients and 90 non-leukemia patients were compared. Results After treatment,the expression level of WT1 gene of the bone marrow cells in all patients were significantly decreased,especially in patients with leukemia,with significant difference（ P〈0.05）. Before treatment,the level of WT1 gene expression in bone marrow cells of patients with leukemia was higher than that in patients with non leukemia with significant difference（ P〈0.05）. After treatment,WT1 gene expression levels in bone marrow cells of patients with leukemia and non leukemia patients were very low with significant difference（ P〉0.05）. Conclusion High expression of WT1 gene in bone marrow cells is one of the indicators for the patients with leukemia; low expression of WT1 gene in bone marrow cells is a good indicator to determine the prognosis of patients with leukemia; low expression of WT1 gene in bone marrow cells doesn＇t rule out the possibility of multiple myeloma or lymphoma in patients,so further examination is needed in clinical practice.
