内镜下治疗食管胃底静脉曲张的临床观察

摘　　要：目的观察内镜下套扎与硬化序贯治疗(EVLS)及胃底组织胶注射治疗对肝硬化食管胃底静脉破裂出血的临床观察。方法选取我院2013年1月~2015年7月的病例,观察105例肝硬化食管胃底静脉曲张出血患者行EVLS及胃底组织胶注射治疗后,食管胃底静脉曲张改善情况,分析治疗的有效性、安全性及并发症。结果经EVLS后,食管静脉曲张完全消失率、基本消失率、改善率分别为24.8%、53.3%、21.9%。胃底组织胶注射后显效83.3%,有效16.7%,6.7%胃底排胶出血。内镜下食管静脉硬化注射术(EIS)后并发出血率1.9%。EIS术后食管狭窄发生率2.9%。结论 EVLS及胃底组织胶注射术治疗效果显著,安全性高,并发症少。objective To observe the clinical curative effect of endoscopic variceal ligation plus sclerotherapy and gastric tissue glue injections in treatment of esophageal and gastrics varices. Methods One hundred and five cases of liver cirrhosis with esophageal Gastric varices bleeding were treated with EVLS and gastric tissue glue injections, the effect, safety and complications were observed. Results After EVLS, the rates of complete Obliteration, basic obliteration and improvement of esophageal varices were 24.8%, 53.5% and 21.9%. After tissue glue injections, gastritic varices narrowed significantly and effectively were83.3%, 16.7%. 6.7% occurred bleeding following gastric Fundal evacuations. The concurrent hemorrhage rate was 1.9% after endoscopic injection sclerotherapy, the rate of esophageal stenoses was 2.9%. Conclusion The effect of the EVLS and the tissue glue injections of gastric venous are remarkable;the higher safety, the less complication, the lower recurrence rates.
