摘  要:目的探讨CT低剂量、低张水灌肠扫描在直肠癌转移淋巴结诊断中的应用价值。方法回顾性分析2014年1月至2015年1月本院收治经病理确诊为直肠癌的50例患者临床资料。所有患者术前均行普通CT扫描(甲方法)以及CT低剂量、低张水灌肠扫描(乙方法)。比较两种检查方法对直肠癌转移淋巴结的诊断价值。结果两种方法诊断N0-3总检出率比较差异无统计学意义(P>0.05);两种方法诊断直肠癌敏感度、特异度及准确性比较差异无统计学意义(P>0.05);两种方法对不同大小淋巴结的检出率比较差异无统计学意义(P>0.05)。结论与常规剂量CT相比,CT低剂量、低张水灌肠扫描在评估直肠癌术前淋巴结分期的准确性基本相当,可降低患者所受到的辐射风险,值得在临床推广运用。Objective To explore the accuracy of and low radiation dose CT scan using water enema in diagnosing lymph node staging in patients with rectal cancer.Methods We performed retrospective analysis of patients admitted to our hospital from January 2014 to January 2015.50 patients who were diagnosed as rectal cancer according to pathology findings were identified.All patients received the routine CT scan and low radiation dose CT scan using water enema.The sensitivity,specificity,and detection rate of lymph nodes in different diameter were compared between the two methods.Results Diagnosis of positive rate of N0-3 was not significantly different between routine CT scan and low radiation dose CT scan using water enema(P〉 0.05).There was no significant difference in sensitivity,specificity and accuracy of diagnosing colorectal cancer between the two methods.(P 〉0.05).There was no significant difference in detecting different sizes of lymph node between the two methods(P 〉0.05).Conclusion Compared with routine CT scans,CT scans with low radiation dose with water enema showed similar capacity in diagnosing lymph node staging in patients with rectal cancer.It has the potential to reduce radiation exposure to the patients.
