长程心理动力学心理治疗对首发缓解期精神分裂症阴性症状的影响

摘　　要：目的观察长程心理动力学心理治疗对首发缓解期精神分裂症阴性症状的影响。方法将14例首发缓解期精神分裂症患者分为研究组和对照组各7例,对照组采用抗精神病药物治疗,研究组在此基础上联合长程心理动力学心理治疗,共随访24周。两组在入组前以及随访结束时,均进行阳性和阴性综合征量表(PANSS)评估。结果随访结束后,研究组的情感迟钝(N1)、情感交流障碍(N3)、抽象思维困难(N5)、交谈缺乏自发性和流畅性(N6)、刻板思维(N7)项目评分及PANSS阴性症状总分较基线时显著降低(P<0.05)。随访结束后,对照组情感迟钝(N1)、抽象思维困难(N5)项目评分均较基线时降低(P<0.05)。随访结束后,研究组情感迟钝(N1)项目评分显著低于对照组(P<0.05)。结论联合长程心理动力学心理治疗有助于改善精神分裂症患者的阴性症状。Objective To explore the effect of long term psychodynamic psychotherapy to treat negative symptoms in patients with first-episode schizophrenia on remission. Method A total of 14 patients with first-episode schizophrenia on remission were randomly divided into study group （n = 7 ） with long term psychodynamic psychotherapy and control group （n = 7 ） without long term psychodynamic psychotherapy for 24 weeks of follow-up. They were assessed with Positive and Negative Syndrome Scare （PANSS） before and after the follow-up. ResultsIn In study group, the factor scores of N1 （Blunted affect）, N3 （Poor rapport）, N5 （Difficulty in abstract thinking）, N6 （Lack of spontaneity and flow of conversation）, N7 （Stereotyped thinking） and total score of negative symptoms of PANSS after the follow-up were all significantly lower than those before the follow-up （P 〈 0.05）. In control group, the factor scores of N1 （Blunted affect） and N5 （Difficulty in abstract thinking） of PANSS after the follow-up were all significantly lower than those before the follow-up （P 〈0.05）. After the follow-up, the N1 （Blunted affect） factor score of PANSS in study group was significantly lower than that in control group （ P 〈 0.05 ）. Conclusion Long term psychodynamic psychotherapy is effective in the treatment of negative symptoms in patients with first-episode schizophrenia on remission.
