摘 要:目的:探讨大鼠肝纤维化不同分期肝表观扩散系数(apparent diffusion coefficient,ADC)与基质金属蛋白酶抑制剂-1(tissue inhibitor of metalloproteinase-1,TIMP-1)mRNA表达的相关性。方法:采用猪血清腹腔注射联合高脂饮食建立肝纤维化大鼠模型。给药4周后开始,取实验组大鼠48只和对照组大鼠12只行磁共振弥散加权成像(diffusion weighted imaging,DWI)检查,计算b值=800 s/mm^2时的ADC值。DWI检查后快速处死大鼠,行病理检查,采用RT-PCR检测TIMP-1 mRNA表达。成模大鼠按肝纤维化病理分期结果又分为S0期组(n=4)、S1期组(n=11)、S2期组(n=12)、S3期组(n=10)和S4期组(n=9),比较肝纤维化各组ADC值及TIMP-1 mRNA的表达,并分析两者的相关性。结果:对照组、肝纤维化S1~4期组大鼠肝ADC值及TIMP-1 mRNA表达差异有统计学意义(分别F=46.54和53.87,均P<0.05),ADC值除对照组与肝纤维化S1期组、S1期组与S2期组、S2期组与S3期组比较差异无统计学意义(均P>0.05)外,其余各组间比较均有统计学意义(均P<0.05);各组TIMP-1 mRNA除肝纤维化S1期组与S2期组、S3期组与S4期组比较差异无统计学意义(均P>0.05)外,其余各组间差异均有统计学意义(均P<0.05)。秩相关分析显示肝ADC值与TIMP-1 mRNA表达的变化呈负相关(r=–0.76,P<0.01)。结论:随大鼠肝纤维化进程不断加深,ADC值逐渐减低,TIMP-1 mRNA表达逐渐升高,二者呈负相关。Objective: To explore the correlation between the apparent diffusion coefficient (ADC) and mRNA expression of tissue inhibitor of metalloproteinase-1 (TIMP-1) in different stages of liver fibrosis in rats. Methods: A model of liver fibrosis in rats was established by intraperitoneal injection of high- fat diet combined with porcine serum. After drug administration for 4 weeks, 48 rats served as a model group and 12 rats served as a control group, then they underwent diffusion weighted imaging (DWI) scanning, The value of ADC was calcuiated at b value=800 s/mm2. The rats were sacrificed and carried out pathologic examination after DWI scanning immediately. The mRNA expression of TIMP-1 was detected by real time-polymerase chain reaction (RT-PCR). The rats of hepatic fibrosis were also divided into a SO group (n=4), a S1 group (n=11), aS2 group (n=12), a S3 group (n=10), and a $4 group (n=9) according to their pathological stage. The value of ADC and the expression ofTIMP-1 mRNA among the different stage groups of liver fibrosis were compared, and the correlation between ADC and the TIMP-1 mRNA were analyzed. Results: The ADC value and the TIMP-1 mRNA expression were significantly different between the control group and the liver fibrosis group (F=46.54 and 53.87, P〈0.05). There were significant differences in the value of ADC between every two groups (all P〈0.05), except the control group vs the S1 group, the S1 group vs the S2 group, and the S2 group vs the S3 group (all P〉0.05). For the comparison of TIMP-1 mRNA, there was no significant difference between the S1 group and the S2 group, theS3 group and the S4 group (both P〉0.05). There were significant differences among the rest of the groups (all P〈0.05). Rank correlation analysis showed that there was a negative correlation between the ADC value and the TIMP-1 mRNA expression (r=-0.76, P〈0.01).
