住院恶性肿瘤患者抑郁障碍的临床特征研究

摘　　要：目的探讨住院恶性肿瘤患者抑郁障碍的发生状况及患者的临床特征，为抑郁障碍的预防提供参考。方法选取我院2014年9月～2016年9月收治的恶性肿瘤住院患者300例作为研究对象，对所有患者的临床资料进行回顾性分析，所有患者均采取问卷调查了解患者的基本资料及患者的抑郁障碍发生状况，并分析总结住院恶性肿瘤患者抑郁障碍的临床特征。结果患者300例中检出抑郁障碍65例，包括心境恶劣31例，重性抑郁障碍34例．抑郁障碍总检出率为21．7％。发生抑郁障碍患者中女性、年龄〈60岁、婚姻状态为未在婚、肿瘤类型为乳腺癌、肿瘤分期为远端转移、病程〈1．5年、治疗方案为姑息治疗的患者发生率与未发生抑郁障碍患者相比明显较高，差异有统计学意义（P〈0．05）。结论住院恶性肿瘤患者抑郁障碍的发生率较高，且住院恶性肿瘤患者抑郁障碍的危险因素较多，临床需加以重视。Objective To provide the instruction for precaution of depression,we explored the prevalence rate and clinical manifestation of malignant tumor patients with depressive disorder in hospital.Methods The study was selected 300 cases of hospitalized patients with malignant tumor as research Object who were collected from Sep.2014 to Sep.2016.We performed a retrospective analysis of clinical data which mainly focused the incidences of accompanied depressive disorder via questionnaire survey.Finally, we analyzed and summarized the clinical features of accompanied depressive disorder on hospitalized patients with malignant tumor.Results Totally,65 cases were diagnosed with depression and the rate was 21.7%,including 1 case of dysthymia and 34 cases of major depression. Compared to malignant tumor patients with depressive disorder, it was indicated that occurrence rates of many factors were obviously higher than that of sole malignant tumor patients,such as female（age〈60）,unmarried,breast cancer, tumor stage of remote metastasis,course of cancer no more than 1.5 years and the treatment of palliative care. Conclusions The depressive disorder was significantly more accessibility to the hospitalized patients with malignant tumor and we attached importance to the numerous risk factors to induce depression.
