虹膜夹型人工晶状体植入术治疗无晶状体眼50例临床分析

摘　　要：目的探讨在无晶状体眼中前房植入Artisan虹膜夹型人工晶状体的疗效和手术安全性。方法回顾性分析50例（50只眼）无晶状体眼患者接受Artisan虹膜夹型人工晶状体二期前房植入后的裸眼视力、最佳矫正视力、眼压、角膜内皮细胞计数、前房深度以及术后并发症等。结果经过6—24个月（平均14．2个月）的术后随访，所有患者术后裸眼视力均较术前有显著提高（术前0．05±0．02，术后0．48±0．09，P〈0．01）。术前、术后最佳矫正视力无显著变化（术前0．47±0．22，术后0．52±0．21，P〉0.05）。术前、术后眼压、角膜内皮细胞计数及前房深度均无明显改变（P〉0．05）。除术后2只眼出现短暂眼压升高，2只眼出现轻度前房渗出反应外，无其他并发症发生。结论Artisan虹膜夹型人工晶状体植入术是一种安全有效的治疗后囊破裂无晶状体眼的方法，手术操作相对简单，组织损伤小，并发症少，可作为常规术式予以考虑。Objective To investigate the therapeutic effects and safety of plantation of iris - clawed intraocular lenses in the anterior chamber of aphakia patients. Methods A retrospective analysis was performed using clinical data from 50 patients （50 aphakic eyes） who were planted with iris - clawed intraocular lenses in the anterior chamber of aphakia, especially uncorrected and optimal corrected eyesights, intraocular pressure, the count of corneal endothelial cells, the depth of the chamber and postoperative complications. Results All the patients were followed up for 6 to 24 months （ 14.2 months on the average） when their uncorrected eyesight was significantly improved compared with that before operation （ before operation 0.05 ± 0.02, after operation 0.48 ± 0.09, P 〈 0.01 ）. No statistical differences were found as to the optimally corrected eyesight （ before operation 0.47 ± 0.22, after operation 0.52 ± 0.21, P 〉 0.05 ）, intraocular pressure, the count of corneal endothelial cells, the depth of the chamber before and after treatment between the two groups （P 〉 0.05）. No complications were reported except two eyes with elevated transient intraocular pressure and another two eyes with slight effusion in the chamber. Conclusions Application of iris - clawed intraocular lenses is a safe and effective approach with simple surgical procedures and less tissue injury and complications.
