泪小管吻合的单泪小管置管与双泪小管小环形置管法疗效比较

摘　　要：目的比较在泪小管断裂吻合术中应用单泪小管置管法和双泪小管小环形置管法的术后疗效和并发症的发生情况。方法收集秦皇岛市海港医院眼科在2009年1月至2016年2月66例（66只眼）下泪小管断裂行手术治疗的患者的临床资料进行研究。采用数字表法随机将患者分为A组和B组。A组应用单泪小管置管，B组应用双泪小管小环形置管。分别采用Mann-Whitney秩和检验和r检验的方法对两组的手术疗效和术后并发症的发生情况进行比较。结果术后A组治愈率39．39％，好转率33．33％，B组治愈率69．69％，好转率21．21％，B组疗效优于A组，差异有统计意义（P〈0．05）；术后A组发生并发症12例，B组发生并发症2例，两组比较差异有统计学意义（P〈0．05）。结论在泪小管断裂吻合术中选择双泪小管小环形置管的方法相比单泪小管置管，术后疗效好，并发症低，值得临床推广。Objective To evaluate the effectiveness and complications by using single canalicular intubation versus bicanalicular small annular intubation in treatment of canalicular laceration. Meth- ods A total of 66 patients with inferior lacrimal canalicular rupture who received surgical treatment by Department of Ophthalmology in Qinhuangdao Haigang Hospital were consecuticvely recruited be- tween January 2009 and February 2016. They were randomly allocated to Group A or Group B. The silicone tube was placed only in the broken canalicular in Group A, and was placed annularly in both the two canaliculars in Group B. Between the two groups, Mann-Whitney Rank sum test was used to comparing the operative effect, and Chi-square test was used to comparing the oc- curence of complications. Results The cure rate was 39.39% in Group A and 69.69% in Group B; the improvement rate was 33.33% in Group A and 21.21% in Group B. Group B had better effect and less complications （2 cases） than Group A （12 cases）, the difference between the two groups were significantly. Conclusions Compared with single canalicular intubation, bicanalicular small annu- lar intubation has better operative effect and less complication occurs, it＇s worthy to be popularized.
