摘 要:目的探讨Calretinin、Melan-A和Inhibinα在肾上腺皮质腺瘤(ACA)组织中的表达及意义。方法采用免疫组化Max Vision两步法检测30例ACA、4例肾上腺皮质结节状增生、30例肾上腺良性嗜铬细胞瘤(ABP)、8例正常肾上腺组织中Calretinin、Melan-A和Inhibinα的表达。结果 Calretinin阳性率:ACA70.0%、肾上腺皮质结节状增生100.0%、ABP 13.4%、正常肾上腺皮质75.0%、正常肾上腺髓质0.0%;Melan-A阳性率:ACA 53.3%、肾上腺皮质结节状增生50.0%、ABP 3.3%、正常肾上腺皮质25.0%、正常肾上腺髓质0.0%;Inhibinα阳性率:ACA 83.3%、肾上腺皮质结节状增生100.0%、ABP 6.7%、正常肾上腺皮质100.0%、正常肾上腺髓质0.0%。Calretinin、Melan-A和Inhibinα在ACA组织中的表达显著高于ABP,差异有统计学意义(0.05)。三者或两两联合检测敏感性最高的是Calretinin与Inhibinα(76.7%)、特异性最高的是Melan-A与Inhibinα(95.0%)、准确性最高的是Calretinin与Inhibinα(83.3%)。结论 Calretinin、Melan-A和Inhibinα联合检测在ACA的诊断及其与ABP的鉴别诊断中具有重要价值。Objective To explore the expressions and significance of Calretinin, Melan-A and Inhibin α in the adrenal cortical adenoma (ACA). Methods The expressions of Calretinin, Melan-A and Inhibin α in 30 cases of ACA,4 cases of adrenal nodular hyperplasia,30 cases of adrenal benign phaeochromoeytoma (ABP) and 8 cases of normal adrenal detected by immunohistochemical MaxVision method. Results Calretinin was positive in the 70.0% of ACA, 100.0% of adrenal nodular hyperplasia, 13.4% of ABP,75.0% of normal adrenal cortical and 0.0% of normal adrenal medulla; Melan-A was positive in the 53.3% of ACA ,50.0% of adrenal nodular hyperplasia ,3.3% of ABP,25.0% of normal adrenal cortical and 0. 0% of normal adrenal medulla; Inhibin α was positive in the 83.3% of ACA, 100.0% of adrenal nodular hyperplasia,6.7% of ABP, 100.0% of normal adrenal cortical and 0. 0% of normal adrenal medulla. The positive expression rate of Calretinin, Melan-A and Inhibin α protein in the ACA was significantly higher than that in the ABP, the difference was statistically significant ( P 〈 0.05 ). Of combined detection of three or two of them, the sensitivity was highest in the Calretinin and Inhibin α (76.7%), the specificity was highest in the Melan-A and Inhibin α (95.0%) ,the accuracy was highest in the Calretinin and Inhibin ct (83.3%). Conclusion The combined detection of Calretinin, Melan-A and Inhibin α has the important practical value in the diagnosis of ACA and the differential diagnosis of ACA and ABP.
