生长抑素联合大黄治疗急性胰腺炎的疗效及抗炎作用

摘　　要：目的:观察生长抑素治疗急性胰腺炎的疗效及其抗炎作用。方法:急性胰腺炎患者52例随机分为对照组和观察组,对照组给予生长抑素等综合治疗,观察组给予生长抑素联合大黄治疗。观察两组腹胀和腹痛消失时间、首次自主排气排便时间及平均住院日。观察两组治疗第1,3,5,7天血浆的肿瘤坏死因子-α(Tumor necrosis factor alpha,TNF-α)、白细胞介素6(nterleukin 6,IL-6)、c反应蛋白(C-reactive protein,CRP)水平,并进行比较。结果:观察组腹胀和腹痛消失时间、首次自主排气排便时间及平均住院时间均短于对照组。观察组TNF-α在第5,7天明显低于对照组;IL-6和CRP在治疗第5天明显低于对照组。结论:生长抑素联合大黄治疗急性胰腺炎疗效显著,可以有效降低急性胰腺炎患者炎性因子TNF-α、IL-6、CPR水平,值得临床推广。Objective： To investigate the curative effect of treating acute pancreatitis with somatostatin combined with rhubarb and its anti -inflammatory effects. Methods： 52 patients wth acute pancreatitis were ran- domly divided into a control group and an observation group. The control group was given somatostatin for the comprehensive treatment, while the observation group was given somatostatin combined with rhubarb. Observed the time of abdominal distension and pain disappearance, the first time of spontaneous exhaust defecation, and the average length of hospital stay in the two groups. Observed and compared the plasma levels of tumor necrosis factor alpha （TNF alpha）, leukocyte mediated factor 6 （IL -6）, C reactive protein （CRP） on the 1st, the 3rd, the 5th , and the 7th days of the two groups. Results： The time of abdominal distension and pain disappearance, the first time of the spontaneous exhaust defecation, and average length of hospital stay in the observation group were shorter than those in the control group. In the observation group, TNF - α was significantly lower than that in the control group on the 5th and the 7th days after treatment, while IL - 6 and CRP were significantly lower than those in the control group on the the 5th day. Conclusion： The curative effect of treating acute pancreatitis with somatostatin combined with rhubarb is remarkable, and it can effectively reduce the levels of inflammatory factors CRP, IL -6 and TNF - alpha in patients with acute pancreatitis, which is worth popularizing in clinic.
